Introduction: The Trifecta bioprosthetic valve has been commonly used for surgical aortic valve replacement (SAVR). Multiple studies have been done to define the rate of structural valve degeneration (SVD) and failure (SVF), but the outcomes are still debatable. Therefore, we aim to conduct this single-center study to estimate the rate and predictors of SVD/SVF.

Background: Duration of dual antiplatelet therapy (DAPT) in patients undergoing percutaneous coronary intervention (PCI) remains uncertain, with increasing data suggestive of acceptable short-term duration. Metabolically accelerated atherosclerosis associated with diabetes makes it essential to study short-term DAPT in this subgroup. With limited studies determining optimal DAPT strategies after second-generation stents in this subset, we aimed to establish the optimal duration of DAPT in the diabetic population using second-generation stents.

Chest pain is one of the most common presenting concerns of patients seeking care in the emergency department, and the underlying etiology can range from acute coronary syndrome to various other non-cardiac causes. Initial evaluation should focus on characterizing symptoms and identifying risk factors, but further risk stratification using clinical decision pathways and biomarkers (cardiac troponin) is essential. The 2021 American Heart Association/American College of Cardiology guidelines for the evaluation and diagnosis of chest pain represent the first ever guidelines for the evaluation of patients with acute chest pain.

Article Synopsis
  • In a study of 318 high-risk atrial fibrillation patients who had left atrial appendage closure, researchers compared the safety and efficacy of direct oral anticoagulants (DOACs) to warfarin post-procedure.
  • The study found no significant difference in the rates of bleeding, thromboembolism, or cardiovascular death within 7 and 45 days between those discharged on DOACs and those on warfarin.
  • Overall, both medications showed similar patient outcomes, with low incidences of complications like major bleeding or device-related issues after the WATCHMAN procedure.

Current guidelines recommend 6-12 months of dual antiplatelet therapy (DAPT) after percutaneous coronary intervention (PCI) followed by aspirin monotherapy indefinitely. We aimed to assess the efficacy and safety of clopidogrel vs aspirin in the post-PCI population after completing DAPT. We systematically searched 5 electronic databases to identify studies comparing clopidogrel with aspirin following completion of DAPT after PCI.

In patient undergoing transcatheter aortic valve implantation (TAVI), stroke remains a potentially devastating complication associated with significant morbidity, and mortality. To reduce the risk of stroke, cerebral protection devices (CPD) were developed to prevent debris from embolizing to the brain during TAVI. We performed a systematic review and meta-analysis to determine the safety and efficacy of CPD in TAVI.

Article Synopsis
  • Ischemic stroke due to atrial fibrillation can be prevented with anticoagulation, but some patients cannot use oral anticoagulants, making the WATCHMAN device a viable alternative that effectively reduces stroke risk.
  • After the WATCHMAN procedure, patients typically receive warfarin and aspirin for 45 days, followed by adjustments based on follow-up tests, with some patients possibly only needing antiplatelet therapy if they have a high bleeding risk.
  • Current practices often favor a regimen combining warfarin and antiplatelets, but more research is necessary to develop tailored antithrombotic strategies for those at elevated bleeding risk.

Article Synopsis
  • - The study investigates right ventricular failure (RVF) after left ventricular assist device (LVAD) surgery, focusing on measuring changes in right ventricle strains using echocardiography techniques.
  • - It analyzed 17 studies, finding that patients experiencing RVF had significantly lower right ventricle global longitudinal strain (RVGLS) and right ventricle free wall strain (RVFWS) compared to those who did not develop RVF.
  • - The results suggest that TTE-derived strains could help predict RVF, while different findings were observed with transesophageal echocardiography (TEE).

Background: This meta-analysis aims to evaluate the utility of speckle tracking echocardiography (STE) as a tool to evaluate for cardiac sarcoidosis (CS) early in its course. Electrocardiography and echocardiography have limited sensitivity in this role, while advanced imaging modalities such as cardiac magnetic resonance (CMR) and 18F-fluorodeoxyglucose positron emission tomography (FDG-PET) are limited by cost and availability.

Methods: We compiled English language articles that reported left ventricular global longitudinal strain (LVGLS) or global circumferential strain (GCS) in patients with confirmed extra-cardiac sarcoidosis versus healthy controls.
