BackgroundKidney impairment (KI) is a frequent complication of multiple myeloma (MM), with chronic kidney disease (CKD) often necessitating dialysis. Peritoneal dialysis (PD) offers quality-of-life advantages over haemodialysis (HD), yet its use in patients with CKD secondary to MM (CKD-MM) remains understudied. This study investigates the characteristics and outcomes of PD in CKD-MM patients compared to those with other kidney diseases.

Of the wide range of symptoms affecting patients with chronic kidney disease (CKD) on haemodialysis, CKD-associated pruritus is one of the most common and burdensome, occurring at moderate-to-severe intensity in 31%-40% of patients, significantly impacting multiple aspects of quality of life, and associated with increased healthcare utilization. Despite the distressing nature of this symptom, clinicians frequently underestimate its prevalence and it is under-reported by patients who may be unaware of the availability of effective treatment options. The identification and management of CKD-associated pruritus should form an essential aspect of patient-centred care; however, patients with CKD may have multiple causes of chronic itch including those of dermatological, systemic, neuropathic and psychogenic origin, and CKD-associated pruritus must be distinguished from these.

Introduction: Discordant results between cystatin C and creatinine in estimating glomerular filtration rate (GFR) are an important medical issue. However, the equation that should be used when GFR estimates are discordant remains unclear.

Methods: This cross-sectional analysis included 15,485 participants with GFR measured by the clearance of an exogenous marker, serum creatinine, and cystatin C.

Home hemodialysis, despite its recognized clinical benefits such as improved cardiac health and enhanced quality of life, remains underutilized worldwide. This study aims to evaluate the efficacy of home hemodialysis within slow daily dialysis programs, employing the Physidia S3 monitor. A prospective trial was conducted with 16 stable patients suffering from end-stage kidney disease and undergoing home hemodialysis.

Background: Ultrafiltration (UF) insufficiency in peritoneal dialysis (PD) often leads to transfer to hemodialysis. Therefore, strategies to prolong time on therapy are needed. The use of a combined crystalloid and colloid solution, also called bimodal PD, can help improve UF.

Key Points: Nurse assistance is associated with a lower risk of transfer to hemodialysis for dialysis inadequacy after 6 months and for infection in the first 18 months. Compared with automated peritoneal dialysis (PD), continuous ambulatory PD is associated with a higher risk of transfer to hemodialysis for mechanical issue during the first 18 months. Suboptimal starters have a higher risk of transfer to hemodialysis due to psychosocial challenges in the first 6 months of PD.

Introduction: Peritonitis occurring within the first months on peritoneal dialysis (PD) has been associated with poorer PD outcomes. Whether early peritonitis is a risk factor for transfer to haemodialysis in the long term is a matter of investigation.

Methods: This retrospective study was conducted using data from the French Language PD Registry of incident PD patients between 2002 and 2018.

Background: Creatinine-based estimated glomerular filtration rate (eGFR) equations are widely used in clinical practice but exhibit inherent limitations. On the other side, measuring GFR is time consuming and not available in routine clinical practice. We developed and validated machine learning models to assess the trustworthiness (i.

Article Synopsis
  • The study evaluates the effectiveness of machine learning (ML) techniques in predicting glomerular filtration rate (GFR) compared to the traditional EKFC equation, which measures kidney function.
  • Using data from 19,629 patients across 13 cohorts, the researchers tested various ML methods, particularly focusing on factors like age, sex, and serum creatinine levels.
  • Results indicated that the random forest (RF) method performed similarly to EKFC, with slight advantages for RF in younger patients, suggesting ML could enhance future GFR prediction methods.

Key Points: It is unknown whether the benefit of assisted peritoneal dialysis (PD) programs appears immediately after PD initiation or rather after some time spent on PD. The protective effect of assisted PD on the risk of transfer to hemodialysis was not constant over time; it started after the first 6 months on PD. Assisted PD programs should be sustainable for at least 6 months to observe their benefits.

  • Onconephrology is a growing field focused on improving care for patients dealing with both cancer and kidney diseases, emphasizing the need for detailed kidney histology data.
  • A retrospective analysis of 154 cancer patients who underwent kidney biopsies showed high rates of complications like proteinuria and kidney injury, with significant histological findings such as interstitial fibrosis and tubular atrophy.
  • The most common cause of kidney issues was found to be nephrotoxicity from anticancer drugs, with many patients having multiple diagnoses, indicating the complex nature of kidney damage in cancer treatment.

Introduction: Several randomised controlled trials have demonstrated that novel oral anticoagulants are safer compared with vitamin K antagonists for the management of non-valvular atrial fibrillation (NVAF) to prevent thromboembolic events in the general population. There is a growing interest in the use of apixaban in patients with end-stage renal disease (ESRD) undergoing peritoneal dialysis (PD) but there is a lack of randomised data in this population.

Methods And Analysis: APIDP2 is a prospective parallel, randomised, open-label, blinded endpoint trial involving patients with ESRD undergoing chronic PD who have NVAF.

Article Synopsis
  • Home dialysis therapies like peritoneal dialysis (PD) and home hemodialysis (HHD) enhance patient quality of life but have low prevalence due to short technique survival times.
  • A retrospective study analyzed data from over 17,000 patients in France to compare the risk of transferring to facility-based hemodialysis among those using autonomous PD, nurse-assisted PD, and HHD.
  • The findings indicated no significant differences in transfer risks among the three modalities, suggesting that assisted PD may benefit a wider range of patients than previously thought, especially in France.

Simulation is a technique to replace and amplify real experiences with guided ones that evoke or replicate substantial aspects of the real world in a fully interactive fashion. In nephrology (a particularly complex specialty), simulation can be used by patients, nurses, residents, and attending physicians alike. It allows one to learn techniques outside the stressful environment of care such as central venous catheter placement, arteriovenous fistula management, learning about peritoneal dialysis, or performing a kidney biopsy.

Article Synopsis
  • - The study investigated how social deprivation affects patients on peritoneal dialysis (PD) in France, using data from a registry that tracked patients from 2017 to 2020.
  • - It found that while the most deprived patients did not have a higher risk of death or combined events like death or transfer to hemodialysis (HD), they did face a greater risk of transferring to HD and had lower chances of getting a kidney transplant.
  • - The analysis showed that social deprivation impacts access to kidney transplantation and increases the likelihood of transferring to HD, indicating disparities in treatment outcomes among different socioeconomic groups.

Chronic kidney disease-associated pruritus (CKD-aP) is a disabling symptom which is frequent and often underestimated. Pa-MRC has a negative impact on quality of life, and is frequently accompanied by sleep disorders and depression. The approval of difelikefalin – a kappa opioid receptor agonist – in this indication requires updated recommendations.
