Purpose: Sleep disturbance is prevalent in long-term care facilities (LTCFs), yet there is limited understanding of individual factors predicting changes in sleep within these populations. Our objective was to determine predictors of sleep disturbance in LTCFs and investigate variation in prevalence across facilities in two Canadian provinces-New Brunswick and Saskatchewan.
Method: This retrospective longitudinal cohort study used interRAI comprehensive health assessment data from 2016 to 2021, encompassing 21,394 older adults aged ≥ 65 years across 228 LTCFs.

Managing diabetes in older adults requires balancing long-term glycaemic control with the prevention of hypoglycaemia, to which this population is particularly vulnerable owing to frailty, multimorbidity and cognitive decline. Guidelines recommend individualized glucose targets for older adults, particularly those with multimorbidity or increased hypoglycaemia risk. For individuals with frailty or cognitive impairment, relaxed HbA1c targets are often appropriate to reduce the risk of adverse events.

Despite its transformative potential, significant challenges persist in optimizing patient identification and referral pathways to ensure timely and equitable access. This expert consensus, developed through the Delphi methodology, analyzes key barriers to the referral process and proposes structured solutions to enhance collaboration between referring treatment centers (RTCs) and qualified treatment centers (QTCs).
