Cryptorchidism is one of the most common reproductive malformations in children, and the timing of surgery significantly impacts fertility and the risk of testicular cancer. Although international guidelines currently recommend testicular fixation within 6-18 months to improve prognosis, many children worldwide undergo surgery later than the recommended age. Delays in surgery are particularly significant in developing countries. This discrepancy may be influenced by factors such as ethnicity, comorbid malformations, economic status, and disease staging. This article provides an overview of global cryptorchidism guideline development and clinical realities, exploring the reasons behind surgical timing disparities to inform the revision of guidelines and improve treatment strategies and prognosis for children with cryptorchidism.

Next-generation sequencing (NGS) has transformed cancer care by providing essential insights for diagnosis, prognosis, and treatment. However, variability in testing timing, reporting practices, and interpretation challenges limits its clinical impact. This manuscript highlights key opportunities to optimize somatic reporting, emphasizing the importance of timely testing throughout the cancer care continuum to maximize the diagnostic and therapeutic relevance of findings.

Paratesticular rhabdomyosarcoma (RMS) is a rare pediatric malignancy often misdiagnosed as benign conditions like epididymitis. We report a 4-year-old boy with paratesticular RMS and retroperitoneal metastasis, initially mistaken for epididymitis. The diagnostic delay resulted in disease progression, necessitating radical inguinal orchiectomy, hemiscrotectomy, and complete multimodal therapy.

Chronic Obstructive Pulmonary Disease (COPD) is a prevalent chronic respiratory disorder characterized by airway inflammation and irreversible airflow limitation. Its marked heterogeneity and complexity pose significant challenges to traditional clinical assessments in terms of prognostic prediction and personalized management. In recent years, the exploration of biomarkers has opened new avenues for the precise evaluation of COPD, particularly through multi-biomarker prediction models and integrative multimodal data strategies, which have substantially improved the accuracy and reliability of prognostic assessments.

Synovial sarcomas are rare malignant soft tissue tumors with significant metastatic potential. Although they can occur in various parts of the body, they are most commonly found on the extremities. These tumors typically develop in children and young adults, making occurrences in individuals over 50 years of age unusual.
