As a widely used vascular access for hemodialysis patients, arteriovenous fistula (AVF) still faces high failure rates, in which local inflammatory response is an essential factor. In animal studies, chronic kidney disease (CKD) has been reported to aggravate local inflammation in AVFs, but the mechanisms are controversial. Here, spatial transcriptomics and single-cell RNA sequencing are used to explore the cellular changes during AVF remodeling in human and mouse. Lymphatic network, facilitated by a group of Pi16Vegfc fibro-progenitors, is revealed as an overlooked efflux tunnel to avoid extensive inflammatory retention in AVFs. In C57BL/6 mice with 5/6 nephrectomy, the elevated phosphorus impaired AVF lymphatic network by increasing soluble VEGFR3 to blunt vascular endothelial growth factor (VEGF)-C sensitivity in lymphatic endothelial cells (LECs), for which the increased SP1/BACE2/VEGFR3 cleavage is the underlying mechanism. By creating LEC specific BACE2 knockout mice or applying BACE2 inhibitors in the anastomotic area, the lymphatic network in 5/6-nephrectomy mouse AVFs is normalized, which alleviated local inflammation and neointima formation. Considering that the hyperphosphatemia is a common metabolic disorder for pre-dialysis CKD patients, this study provides a novel immunoregulation strategy for AVFs under CKD condition, as suppressing BACE2-mediated VEGFR3 cleavage to recover a functionally competent lymphatic network is a potential target.

Chylous ascites from small bowel obstructions is a very rare finding with only a handful of case reports previously published. This case report of a patient with chylous ascites related to an obstruction from Petersen's hernia supports the trend from existing reports. Prior studies have linked chylous ascites to closed-loop obstructions, such as small bowel volvulus or internal hernia, even when the bowel is viable and does not require resection.

The Chinese Society for Therapeutic Radiology Oncology, the Chinese Anti-Cancer Association, the Chinese Society of Clinical Oncology, the Head and Neck Cancer International Group, the European Society for Radiotherapy and Oncology, and the American Society for Radiation Oncology collaboratively developed evidence-based guidelines and a comprehensive contouring atlas for neck target volume delineation in nasopharyngeal carcinoma. These guidelines address five key challenges in modern radiotherapy practice: margin design of clinical target volume; nodal target volume delineation after induction chemotherapy; delineation of equivocal nodes evident on imaging; low-risk clinical target volume delineation based on regional stepwise extension patterns; and modifications for anatomical boundaries of lymphatic areas. Developed through a rigorous systematic review and expert appraisal process by a panel of 50 international, multidisciplinary members from 17 countries and regions, these guidelines incorporate the latest advances in nasopharyngeal carcinoma diagnosis and treatment.

Lymph node metastasis is crucial for esophageal squamous cell carcinoma (ESCC) malignancy. However, the molecular drivers and related mechanisms of lymph node metastasis in ESCC cells are unclear. In the present study, we found that the tyrosine kinase complex-focal adhesion kinase (FAK)/Src family kinase (SFK) axis specifically contributes to metabolic reprogramming by inducing the phosphorylation of ATP-citrate synthase (ACLY) Tyr542, Tyr652, and fructose-bisphosphate aldolase A (ALDOA) Tyr174, Tyr302, or Tyr328 sites in both primary and metastatic ESCC cells.
