Article Abstract

Calcium release through the cardiac RyR2 (type-2 ryanodine receptor) is essential for cardiac contraction. RyR2 dysfunction is associated with a spectrum of cardiac pathologies, most notably arrhythmias. While excessive RyR2 activity was historically seen as the driver of arrhythmia, it is now clear that inadequate calcium release is equally detrimental. This homeostatic balance of activity requires precise tuning, which is provided by a swathe of regulating factors spanning posttranslational modifications, protein-protein interactions, and the more recently identified positioning of individual RyR2 channels within the cell. This review summarizes how too much, or too little, calcium release can lead to arrhythmia and explores how the multitude of regulating factors work synergistically to set and modify RyR2 physiologically and become impaired in disease. Finally, we examine how RyR2-targeted pharmacological approaches can therapeutically rebalance calcium handling and inhibit arrhythmia.

While reactive oxygen species (ROS)-dependent chemodynamic therapy (CDT) and photodynamic therapy (PDT) hold promise for cancer treatment, their efficacy remains constrained by tumor microenvironment (TME) barriers: glutathione (GSH) overexpression, insufficient HO supply, and hypoxia. To address these limitations, we engineered a Trojan horse-inspired MnO-shelled CaO nanoreactor (CaO/MnO-Ce6-PEG) by employing a sequential TME reprogramming strategy, triggering a cascading ROS storm for enhanced CDT and PDT. The outer MnO layer first depletes GSH through redox conversion, exposing the CaO core hydrolysis, and subsequently providing HO for CDT and O for ameliorating hypoxia to boost Ce6-mediated PDT.

Cognitive impairment represents a progressive neurodegenerative condition with severity ranging from mild cognitive impairment (MCI) to dementia and exerts significant burdens on both individuals and healthcare systems. Vascular cognitive impairment (VCI) represents a heterogeneous clinical continuum, spanning a spectrum from subcortical ischemic VCI (featuring small vessel disease, white matter lesions, and lacunar infarcts) to mixed dementia, where vascular and Alzheimer's-type pathologies coexist. While traditionally linked to macro- and microvascular dysfunction, the mechanisms underlying VCI remain complex.

The activation of nucleotide oligomerization domain-like receptor (NLR) family, pyrin domain-containing protein 3 (NLRP3) inflammasome is implicated in the pathogenesis of various inflammatory diseases. The natural product oridonin possesses a novel mechanism for NLRP3 inhibition and a unique binding mode with NLRP3, but its poor anti-inflammatory activity limits further application. After virtual screening of diverse natural product libraries, dehydrocostus lactone (DCL) was considered as a potential NLRP3 inhibitor.

The indications for extracorporeal membrane oxygenation (ECMO) have broadened in clinical practice, and its use in circulatory failure caused by acute drug intoxication has become more frequent. We reviewed three cases of venoarterial (VA) ECMO use for intoxication at our hospital.
