Article Abstract

Background: The Epidemiological Unit of the Agency for Health Protection of Milan (ATS) calculates several indicators, at hospital and at patients' residence area level (group level of analysis), with monitoring and programming scopes. Outcome indicators are usually influenced by differences in subject case-mix; therefore, adjustment methods are applied to compare each group level with mean ATS values. Inverse probability weighting (IPW) is explored as an alternative to multivariable generalized linear model (GLM), to overcome some limitations of the latter method.

Methods: To implement IPW, a multinomial logistic model, with group level as dependent and subject characteristics as independent variables, was used to estimate patient weights, which were subsequently stabilized and truncated. Checks on IPW assumptions and covariates balance were implemented both in a quantitative and in a graphical way. Comparisons on adjustment performed with fixed effects and random intercept multivariable GLM were performed and exemplified using three outcome indicators.

Results: IPW assumptions were satisfied for all the indicators, and covariate balance presented minor issues for group levels with the lowest number of cases/events. Case-mix adjustment performed with multivariable fixed effects GLM showed a tendency to overestimate raw values and was characterized by broad confidence intervals in the three case-example indicators. Adjusted values produced using random intercept multivariable GLM suffered from a shrinkage towards the mean effect, particularly evident in the indicators with the lowest number of cases. IPW-adjusted estimates differed from raw values only when substantial differences in covariates distribution were present.

Conclusions: Provided that appropriate checks are implemented, IPW adjustment is applicable in the context of healthcare quality evaluation and can be easily conveyed to healthcare managers for effective dissemination.

The effectiveness and tolerability of medicines can vary considerably from person to person, even at the same dose. This variation is influenced by many factors, including constitutional genetic characteristics. In fact, some people have genetic variations that are common and neutral in the population, known as polymorphisms, which can affect drug metabolism or make them more susceptible to certain adverse effects.

Is the use of intraoperative vasopressors associated with flap failure in head and neck free tissue transfer surgery?

Background: Maintaining appropriate blood pressure during head and neck free tissue transfer surgery is important for both organ and flap perfusion. However, the use of vasopressors to treat intraoperative hypotension is controversial. The purpose of this prospective cohort study is to evaluate the impact of intraoperative vasopressors on the incidence of flap necrosis.

Background: Advanced glycation end-products result from chemical modification of proteins under conditions of hyperglycemia or oxidative stress common with advancing age. Advanced glycation end-product (AGE) formation alters vascular and cardiac structure and function, yet the prospective associations of circulating AGEs with heart failure (HF) and atrial fibrillation (AF) have not been studied.

Methods: We evaluated the associations of serum N-carboxymethyl-lysine (CML), a major AGE in tissue proteins, and incident HF and AF in the CHS (Cardiovascular Health Study), a population-based cohort of older adults.

Background: Obesity has emerged as a critical global public health challenge. Postmenopausal women experience significantly elevated risks of metabolic disorders and a marked increase in obesity prevalence due to declining estrogen levels. The uric acid to high-density lipoprotein cholesterol ratio (UHR), an emerging biomarker for metabolic syndrome, is gaining clinical recognition.

Background: Musculoskeletal disease (MSD), including osteoarthritis, rheumatoid arthritis, osteoporosis, and sarcopenia, poses a serious social burden. While physical activity (PA) benefits musculoskeletal health, the optimal PA level for MSD prevention remains unclear. Clarifying risk factors and biological mechanisms is essential.
