Article Abstract
Surgical resection of the caudate lobe of the liver remains the final hurdle for liver surgeons, not only in open hepatectomy but also in recent minimally invasive hepatectomy. In the dawn of liver surgery, Prof. Kumon made hepatic casts and showed the anatomy of the caudate lobe of the liver based on the portal segmentation in the National Cancer Center Hospital, Tokyo. Meanwhile, liver surgeons in the center successfully performed isolated caudate lobectomy of the liver for liver cancers one after another. Prof. Kumon is still dissecting hepatic casts to demonstrate the right border of the paracaval portion of the caudate lobe against segment VIII of the liver. An approach to right hemihepatectomy preserving the paracaval portion of the caudate lobe was developed thanks to the detailed anatomical knowledge of the liver based on hepatic casts.

Identifying the right border of the caudate lobe against the right liver is clinically important; however, this remains challenging. As the paracaval portion (PC) of the caudate lobe is adjacent to segment 8 of the right liver, we dissected a liver cast made from epoxy resin and colored dye to define the right border of the PC against segment 8. On the right border of the PC, two major venous plexuses appearing as bouquet-shaped branches joined the inferior vena cava and the middle hepatic vein, forming short hepatic veins, whereas the venous plexuses in segment 8 joined the right hepatic and the vein inferior vena cava.

Background: Liver mobilization is essential for exposing the retrohepatic inferior vena cava (IVC) during level II-III robot-assisted IVC thrombectomy (RA-IVCT), but complex cases present significant challenges with a high risk of severe complications. This study aimed to evaluate the safety and feasibility of caudate lobectomy in facilitating retrohepatic IVC exposure in these complex cases.

Castleman disease (CD) is a group of rare lymphoproliferative disorders characterized by shared histopathological features but distinct clinical entities, broadly classified into unicentric Castleman disease (UCD) and multicentric Castleman disease (MCD). UCD involves a single anatomical site and typically follows a benign clinical course, whereas MCD affects multiple lymph node stations and is associated with systemic symptoms and a more complex therapeutic approach. The disease is poorly understood, and the difficulty in reaching a diagnosis is well noted in the literature.
