Article Abstract

Background: Iatrogenic injury of the parathyroid gland during thyroid surgery cannot be completely avoided. The lateral approach is the most commonly used surgical approach in traditional thyroid surgery, whereas the complete intermediate approach has become common in endoscopic surgery and has been applied in traditional open surgery. In this study, the primary aim was to compare the effects of two surgical approaches on postoperative parathyroid function.

Methods: This study enrolled and analyzed 270 patients with papillary thyroid cancer. The patients underwent total thyroidectomy between January 2020 and December 2022 at three medical centers and were confirmed to have papillary thyroid cancer by paraffin pathology. All the patients were diagnosed by pathologists on the basis of preoperative and postoperative specimens. The sex, age, tumor diameter, lymphatic dissection range, operation time, preoperative parathyroid hormone level, parathyroid hormone level during follow-up, etc., were analyzed. Parathyroid function was monitored at 6 months after the surgery to identify hypoparathyroidism as permanent.

Results: The enrolled patients were divided into two groups: the complete intermediate approach surgical protocol group (the observation group) and the traditional thyroid surgery protocol group (the control group). There was no significant difference in preoperative blood parathyroid hormone levels or serum calcium levels between the observation group and the control group. However, the blood parathyroid hormone levels and blood calcium levels in the observation group were significantly greater than those in the control group on the first day after surgery. During the follow-up, these levels in all the patients in the observation group returned to the normal range, whereas four patients in the control group still exhibited decreased parathyroid hormone levels at the three-month and six-month follow-up, and thus was diagnosed as permament hypoparathyroidism.

Conclusion: Compared with the traditional lateral approach, the complete intermediate approach better protects parathyroid function and reduces the incidence of iatrogenic parathyroid injury.

The Application of Assay- and Age-Specific Parathyroid Hormone (PTH) Reference Intervals Decreases the Diagnosis of Normocalcaemic Primary Hyperparathyroidism and Improves Diagnostic Concordance of PTH Assays.

Parathyroid hormone (PTH) assays are not standardized and therefore PTH results are interpreted using manufacturer-provided assay-specific reference intervals. Assay-specific PTH reference intervals, however, do not account for between-assay differences and lead to discordance in the diagnosis of normocalcaemic primary hyperparathyroidism (NCPHPT). PTH increases with age independent of vitamin D, renal function, phosphate and ionized calcium.

Background: The primary goals of surgical management in thyroid disorders are the treatment of malignancy, goiter, and thyrotoxicosis, while adhering to the principle of 'primum non nocere'. Hypocalcemia is among the most common complications, primarily resulting from inadvertent injury to the parathyroid glands.

Methods: A retrospective analysis of 2015-2023 records of Polish pediatric patients who underwent thyroid surgery.

Department of Endocrinology and Metabolism, Jiangxi Medical College, The Second Affiliated Hospital, Nanchang University, Nanchang City, 330006, Jiangxi Province, China.

Objective: To investigate the association between thyroid hormone sensitivity indices and bone metabolism markers in newly diagnosed middle-aged and elderly type 2 diabetes mellitus (T2DM) patients with normal thyroid function.

Method: We retrospectively analyzed 350 newly diagnosed T2DM patients (≥ 45 years), stratified by bone mineral density into Group A (normal bone density group) and Group B (low bone mass and osteoporosis group). General data and clinical biochemical parameters were collected: free triiodothyronine (FT3), free thyroxine (FT4), thyroid-stimulating hormone (TSH), 25-hydroxyvitamin D (25(OH)D), parathyroid hormone (PTH), osteocalcin (OC), bone-specific alkaline phosphatase (BALP), serum calcium (Ca), serum phosphorus (P), fasting plasma glucose (FPG), glycosylated hemoglobinA1c (HbA1c), total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), serum creatinine (SCr), serum uric acid (SUA), and estimated Glomerular Filtration Rate (eGFR).

Background: In maintenance hemodialysis (MHD) patients, vascular calcification can be detected not only in coronary vessels but also in ocular areas. However, ophthalmic examinations are not sufficiently validated to measure the degree of vascular calcification.

Methods: This study was performed prospectively, involving 32 MHD patients.
