Background: Thirteen cases of fungal cavernous-carotid aneurysms (CCAs) attributable to invasive fungal sinusitis have been reported in the literature. These reports included only 1 case of carotid-cavernous fistula (CCF). In this report, the authors present a case of CCF attributable to the rupture of an infected CCA caused by invasive fungal sinusitis that required proximal occlusion for treatment.
Observations: A 56-year-old man with poorly controlled diabetes experienced a sudden loss of vision in his left eye. Cranial imaging revealed invasive sphenoid sinusitis and CCF attributable to a ruptured infected CCA, as well as an unruptured infected supraclinoid internal carotid artery (ICA) aneurysm. After 3 weeks of medical treatment, the patient developed sudden impairment of higher function. MRI revealed occlusion of the distal ICA attributable to thrombosis of the supraclinoid ICA aneurysm. The anterior circulation was perfused from the contralateral side via the anterior communicating artery. Proximal occlusion with coils was performed, leading to ablation of the CCF.
Lessons: This is the first reported case of spontaneous thrombosis of an infected distal cerebral aneurysm, and proximal occlusion with coils was necessary to treat CCF caused by the rupture of a fungal CCA. This case could help guide future treatment. https://thejns.org/doi/10.3171/CASE25336.

Objective: To evaluate the impact of occlusion type and artificial intelligence-based computer-aided design (CAD) software on the geometric accuracy and clinical quality of auto-generated anterior and posterior crown designs.
Methods: Five typodont models representing various occlusion types (normal, Class I anterior diastema, Class II division 1, Class II division 2, and Class III anterior crossbite occlusion) underwent crown preparation for the maxillary right central incisor and first molar. Ten sets of intraoral scans were obtained from each prepared model, and crown designs were automatically generated using two software programs: deep learning-based (DL; Dentbird) and conventional automated (CA; Auto Workflow, 3Shape) (n = 10).

Wellens' syndrome is characterized by a distinct electrocardiographic pattern, most notably biphasic or deeply inverted T waves in the anterior precordial leads, particularly V2 and V3. These findings typically reflect transient myocardial ischemia resulting from critical stenosis of the proximal left anterior descending (LAD) artery. They are often a warning sign of an impending anterior wall myocardial infarction.

Background: The de Winter electrocardiographic (ECG) pattern, characterized by upsloping ST-segment depression and tall T waves in leads V1-V6, is typically associated with proximal left anterior descending (LAD) artery occlusion. When combined with pronounced ST-segment elevation in lead aVR, it may indicate a more severe coronary artery involvement.

To evaluate and compare pregnancy outcomes following successful fluoroscopy-guided tubal recanalization (FGTR), focusing on spontaneous conception versus intrauterine insemination (IUI). This retrospective cohort study included 139 women aged 21-40 years who underwent FGTR for tubal occlusion between January 2021 and May 2024. After exclusions, 80 women attempted natural conception, and 59 underwent IUI with ovarian stimulation.
