Introduction:  Taxanes, including paclitaxel, docetaxel, and cabazitaxel, are widely used anticancer agents that disrupt cell division by binding to microtubules, but are associated with significant adverse reactions, particularly infusion-related reactions (IRRs), such as flushing, urticaria, and respiratory symptoms. Despite premedication with steroids, antihistamines, and antiemetics per guidelines, taxane-induced side effects remain prevalent and can result in treatment delays or discontinuation, impacting patient outcomes. This study aimed to observe and document the incidence and spectrum of adverse reactions to taxanes among premedicated cancer patients to improve management and overall chemotherapy success.

Rationale: Primary polydipsia refers to excessive water intake due to psychogenic or non-psychogenic causes without being secondary to conditions such as hyperglycemia or renal dysfunction. Most cases of primary polydipsia are psychogenic in nature, with few cases of non-psychogenic primary polydipsia reported in the literature. In this case, the patient's excessive water intake appeared to be influenced by both psychogenic and non-psychogenic factors.

Multiple exogenous supplements to achieve ketosis using the oral route have been developed to elevate blood BHB levels on demand and in a controllable fashion. The focus is now shifting to evaluating these supplements as potential therapeutic agents and developing strategies to not only achieve ketosis but also maintain it. One such strategy is to administer these as a continuous IV infusion.

Objectives: A framework of high-reliability principles was used to identify, investigate, and mitigate infusion pump safety concerns at a large, multisite health care system. We developed a systematic approach to address challenges associated with overinfusions, underinfusions, and the inability to clear upstream occlusion alarms. We identified 112,875 upstream occlusion events for 389,604 infusion starts (failure rate, 29%) within 6 months.
