We report a 28-year-old woman with refractory hypoglycemia, hypotension, and profound fatigue found to have panhypopituitarism secondary to Sheehan syndrome. Although she had a remote history of postpartum hemorrhage marked by agalactia and secondary amenorrhea, her diagnosis was delayed until she developed an adrenal crisis in the setting of acute pyelonephritis. Comprehensive endocrine testing confirmed secondary adrenal insufficiency, central hypothyroidism, hypogonadotropic hypogonadism, and lactotroph failure; Magnetic resonance imaging demonstrated a partially empty sella consistent with remote pituitary infarction.

Critical cardiac illness in the setting of pregnancy presents a unique and complex challenge requiring multidisciplinary expertise and coordination. Physiologic changes of pregnancy can unmask or exacerbate underlying cardiac disease, placing both patient and fetus at significant risk. This review provides a comprehensive approach to the management of critically ill pregnant individuals with cardiac disease within the cardiac intensive care unit.

Mechanistic relationships between heat shock protein 90 (HSP90) and human epidermal growth factor receptor 2 (HER2) are complex and clinical correlations in breast cancer remain inconsistent. We investigated the role of HSP90 expression in the response of breast cancer cells to HER2-targeted treatments, by measuring cell viability/proliferation and protein expression after genetic and pharmacologic HER2/HSP90 modulation. HSP90 expression was also assessed by immunohistochemistry in a series of 72 metastatic, HER2+ breast cancer patients.
