A vertebral telangiectatic osteosarcoma was diagnosed in a 5 yr old female neutered Staffordshire bull terrier presenting with paraplegia with nociception in one limb and absent on the other. Computed tomography revealed a hypoattenuating, osteolytic lesion affecting the T9 vertebra. MRI revealed a well-demarcated dorsal extradural mass overlying the spinal cord along the T8-T10 vertebral bodies. A hemorrhaging, reddish mass dorsal to the spinal cord was removed surgically, and a diagnosis of telangiectatic osteosarcoma was confirmed through histopathological examination. The dog improved following surgery and chemotherapy but suffered acute deterioration 5 wk postoperatively and was euthanized. This is the first reported case of a vertebral telangiectatic osteosarcoma where computed tomography and MRI sequences were obtained, with surgical extraction and medical chemotherapy. This subtype of osteosarcoma is believed to have a negative prognosis in dogs, although this is the first report of attempted surgical treatment. Telangiectatic osteosarcoma should be considered as a differential in dogs, particularly when a mass is identified with associated intralesional hemorrhagic spaces, commonly seen in the telangiectatic subtype. Currently, there are no established treatment options for vertebral telangiectatic osteosarcomas and the prognosis appears to be poor.

This case report is focused on a patient (a 32-year-old previously healthy man) with a femoral bone tissue tumor. The case underscores the diagnostic complexity of Telangiectatic Osteosarcoma (TOS) with Aneurysmal Bone Cystic (ABC) lesion and the importance of maintaining special immunohistochemical markers for differential diagnosis between low-grade osteosarcoma, primary ABC, and suspected TOS, which is a true mimicker of ABC. On the other hand, this presentation describes some molecular rearrangements seen in ABC.

Telangiectatic osteosarcoma (TOS) is a rare high-grade malignant tumour found primarily in the metaphysis of long bones, but can also occur in the pelvis, spine and skull. Characterized by extensive vascular dilatation and aggressive behavior, TOS often has a poor prognosis and may be misdiagnosed due to similarities with other lesions. This report presents a case of TOS of the thoracic vertebrae and reviews the relevant literature to enhance clinical awareness.

Introduction And Importance: Telangiectatic osteosarcoma (TOS) is a rare and aggressive subtype of osteosarcoma that is often misdiagnosed as more benign lesions, such as aneurysmal bone cysts (ABCs) or giant cell tumors (GCTs). The accurate differentiation between these conditions is crucial to ensure timely and appropriate treatment, as misdiagnosis can lead to delayed management and poor prognoses.
Case Presentation: We present the case of a 12-year-old girl who initially presented with left hip and proximal thigh pain, with imaging studies suggesting an ABC.
