Article Abstract
It is still challenging to diagnose biliary atresia (BA) in current clinical practice. The study aimed to develop a multimodal model incorporated with uncertainty estimation by integrating sonographic images and clinical information to help diagnose BA. Multiple models were trained on 384 infants and validated externally on 156 infants. The model fused with sonographic images and clinical information yielded best performance, with an area under the curve (AUC) of 0.941 (95% CI: 0.891-0.972) on the external dataset. Moreover, the model based on sonographic video still yielded AUC of 0.930 (0.876-0.966). By excluding 39 cases with high uncertainty (>0.95), accuracy of the model improved from 84.6% to 91.5%. In addition, six radiologists with different experiences showed improved diagnostic performance (mean AUC increase: 0.066) when aided by the model. This fusion model incorporated with uncertainty estimation could potentially help radiologists identify BA more accurately and efficiently in real clinical practice.

Importance: The association of cardiopulmonary point-of-care ultrasonography (POCUS) with length of stay (LOS) and hospitalization costs for patients admitted to internal medicine wards remains uncertain.
Objective: To evaluate a collaborative implementation model involving hospitalists, sonographers, and a remote cardiologist for integrating cardiopulmonary POCUS into the assessment of adult patients (≥18 years) hospitalized with undifferentiated dyspnea, and to assess its association with LOS and hospitalization costs.
Design, Setting, And Participants: This quality improvement study employed a type 1 effectiveness-implementation hybrid design using a 6-month stepped-wedge cluster randomized approach, conducted at a tertiary care hospital in the US between December 7, 2023, and July 2, 2024, to compare the standard-of-care (control) with the intervention group.

On examination, ISC is characterized by painless, firm nodules and papules localized to the scrotal skin. Transcranial sonography (TCS) is widely acknowledged as a frontline imaging tool in movement disorder practice, particularly for separating idiopathic Parkinson's disease from its many mimics. In recent years, however, investigators have extended its reach, showing that the same portable probe can also capture structural and hemodynamic signatures of neuropsychiatric disorders and the major dementia syndromes. Across neuropsychiatry, a dim ("hypoechoic") median raphe emerges as the sonographic hallmark of serotonergic imbalance: it recurs in major depressive disorder, bipolar depression, and panic disorder, predicts better response to selective serotonin reuptake inhibitors, and even foreshadows post-stroke depression.
