This review article presents the spectrum of pharmacological action and aspects of clinical application of the original domestic drug Dimephosphon (international nonproprietary name - dimethyl oxobutylphosphonyl dimethylate) based on a significant number of sources. The article reflects the results of experimental and clinical studies on the efficacy and safety of Dimephosphon, indicating its multimodal action and the possibility of using the drug as a means for normalizing autonomic homeostasis.

Objective: To evaluate the efficacy of the intravenous form of dimephosphon at the first stage of rehabilitation in patients with ischemic stroke in the carotid system.
Material And Methods: The article presents the results of examination of 60 patients in the acute period of ischemic stroke in the carotid system (30 patients were treated with dimephosphon and 30 patients were in the comparison group). The dynamics of the clinical picture (according to the NIHSS), functional dependence in self-service (according to the modified Rankin scale), state of autonomic response using spectral analysis of heart rate variability, cerebrovascular reactivity using functional tests were assessed during the treatment.

The condition of the mucous membrane and lipid spectrum of tissue structures under the action of gastric ulcerogenic stress factors was studied on the background of administration of vitamin E, dimephosphon, and reamberin. The use of antioxidants, especially reamberin, led to the restoration of lipid metabolism and normalization of lipid peroxidation processes.

The Freund adjuvant induced chronic arthritis and increased the concentrations of PLO products and nitrite ions in the blood plasma. Only dimephosphone showed an anti-inflammatory action.

Gastroprotective activity of dimephosphon, xydiphone, and ionol and their effects on the lipid peroxidation (LPO) indices and the activity of antioxidant enzymes in blood and organ tissues were studied on the model of indomethacin-induced gastric lesions in white rats. The indomethacin-induced gastropathy leads to increased concentration of LPO products in blood plasma and tissues. Dimephosphon and ionol reduced the number of gastric lesions and prevented LPO activation.
