Background: Cystic fibrosis (CF) is a genetic disorder that remains underrecognized across Africa, where limited diagnostic capacity, low awareness, and competing health priorities contribute to delayed or missed diagnoses [1-4]. Although increasing data suggests CF is more prevalent than previously believed in Africa, survival remains poor [1]. Maternal undernutrition in Africa remains a public health challenge, contributing to negative pregnancy outcomes, neonatal mortality, and perpetuating intergenerational cycles of poor health. Antenatal multiple micronutrient supplementation (MMS), a cost-effective intervention recognized for its potential to improve maternal and neonatal health, reduces risks of low birth weight, preterm birth, small for gestational age, and stillbirth while offering a $37 return for every $1 invested. Despite its benefits, MMS adoption across African countries remains suboptimal.

Amid increasing political threats to sexual and gender minority (SGM) research, this perspective reflects on the evolution of SGM research from the margins of public health to a robust field and assesses the unique vulnerabilities facing SGM researchers across career stages. We discuss how the field can protect its infrastructure, sustain its workforce, and preserve its impact, drawing inspiration from the harm reduction movement and our individual and collective experiences. This is both a reflection and a call to action grounded in care, intergenerational knowledge and solidarity, and the refusal to be erased.
