Article Abstract

This study described knowledge and usage of doxycycline postexposure prophylaxis (doxyPEP) among preexposure prophylaxis users. A survey was launched enrolling 703 respondents: 28.2% were aware of doxycycline postexposure prophylaxis, but only 8.8% used it. One quarter discussed it with medical doctors, and only 51.6% used the recommended dosing. Doxycycline postexposure prophylaxis needs to be implemented within appropriate guidelines.

Clinical trials show that doxycycline post-exposure prophylaxis (DoxyPEP) significantly reduces the risk of bacterial sexually transmitted infections (STIs), especially syphilis and chlamydia, in cisgender men who have sex with men and transgender women. Real-world data suggest DoxyPEP may be beneficial for other populations disproportionately affected by STIs. Given the ongoing STI epidemic we recommend discussing DoxyPEP with all eligible patients.

Objectives: Doxycycline postexposure prophylaxis (DoxyPEP) has emerged as a promising tool for reducing bacterial sexually transmitted infections (bSTIs), particularly among men who have sex with men. This study aims to investigate the attitudes and prescribing practices of Italian infectious disease (ID) physicians regarding DoxyPEP for the prevention of bSTIs.

Methods: This is a cross-sectional survey conducted between October 2024 and March 2025 among ID specialists registered with the Italian Society of Infectious and Tropical Diseases.

The incidence of bacterial sexually transmitted infections (STIs) has increased over the past decades to unprecedented figures, and has become a worldwide public health problem. While men who have sex with men (MSM) and transgender women (TGW) are particularly affected, the most detrimental consequences concern cisgender women, and in particular pregnant women. Behavioral modifications have had limited effects so far and vaccines are not available yet, leaving an opportunity for antibiotic prophylaxis.

We iteratively designed a novel patient decision aid (PDA) to improve patient-provider communication regarding the benefits, risks, and proper usage of doxycycline as post-exposure prophylaxis (Doxy-PEP). We tested the feasibility, acceptability, and appropriateness of counseling using this PDA among a pilot group in two academic, urban sexual health clinics. Using a pretest-posttest model we assessed the extent to which this intervention impacted participants' knowledge of and attitudes towards Doxy-PEP.
