A 75-year-old woman presented with scalp and cranial defects caused by implant exposure, infection,and cranial necrosis following multiple craniotomies. To reduce the risk of complications after cranioplasty, weplanned a two-stage cranial reconstruction. A wide bipedicled scalp flap with a long longitudinal axis above the cranium covered the scalp defect after debridement, and skin grafting was performed on the periosteum after flap harvesting during the first surgery. A custom-made titanium plate was implanted under the bipedicled flap during the second cranioplasty 4 months after the initial surgery. The patient had no complications during the two surgeries, and her clinical course was uneventful for 6 months after the last surgery. Although a two-stage approach, our technique is relatively safe for patients who have previously undergone multiple surgeries and decreases the risk of postoperative complications due to the shorter exposure time of the titanium plate during the second surgery. A retrospective analysis of 47 consecutive patients treated between January 2022 and June 2024 documented operative time, intraoperative blood loss, fluoroscopy times, healing time, complications, postoperative fracture gap distance (mm), and articular surface step-off (mm) at the ankle joint. Ankle function was assessed using the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score, the Manchester-Oxford Foot Questionnaire (MOXFQ), and range of motion measurements at 3, 6, 12, and 14 months.

Segmental mandibulectomy is a valid treatment for advanced medication-related osteonecrosis of the jaw (MRONJ), but subsequent reconstruction is challenging. Spontaneous bone regeneration (SBR) following resection is exceedingly rare, particularly in the elderly. The patient-specific mandibular reconstruction plate (PSMRP) has gained prominence for its precise adaptation to mandibular contours and reported enhanced mechanical reliability compared to the manual-bent mandibular reconstruction plate (MBMRP). Despite this, a biomechanical comparison between MBMRP and PSMRP is essential for informed clinical decision-making and advancing the field of mandibular reconstruction. Thereupon, biomechanical behavior was compared between two reconstructed mandibles with the MBMRP made of commercial pure titanium plate and the PSMRP stemmed from a common ramus reconstruction case, respectively, in physiological simulation using finite element analyses.
