Objectives: To investigate whether renal function impacts CSF κ-FLC concentration and/or κ-FLC index.
Methods: Patients with non-inflammatory neurological diseases were eligible. κ-FLC index was calculated as (CSF κ-FLC/serum κ-FLC)/albumin quotient. Structural equation modeling (SEM) was used to evaluate the direct influence of GFR on serum κ-FLC concentration and albumin quotient (Q), and via these two variables the indirect influence on CSF κ-FLC concentration.
Results: A total of 129 patients with a median age of 65 years and 42 % females were included. κ-FLC index ranged from 0.57 to 3.56 and glomerular filtration rate (GFR) ranged from 17 to 128 mL/min/1.73 m. While a correlation of GFR with CSF κ-FLC concentration was observed (r= -0.52, p<0.001), there was no statistically significant correlation with κ-FLC index (r=0.14, p=0.113). SEM revealed that higher age was associated with lower GFR (β= -0.53), which led to higher serum κ-FLC concentration (β= -0.45) and higher Q (β= -0.17), while CSF κ-FLC concentration increased with serum κ-FLC concentration (β=0.75) and Q (β=0.39), indicating that GFR did not directly influence CSF κ-FLC concentration (RMSEA=0.043).
Conclusions: CSF κ-FLC concentration is not directly affected by renal function. The κ-FLC index compensates for renal function effects by factoring in serum κ-FLC concentration and Q. κ-FLC index can be interpreted without considering renal function.

Background And Objectives: Myelitis is a relatively common clinical entity for neurologists, with diverse underlying causes. The aim of this study was to describe the incidence of myelitis, its causes, clinical presentation, and factors predicting functional outcomes and relapses.
Methods: Using the Swedish National Patient Registry, we identified all adult patients in Stockholm County between 2008 and 2018 using International Classification of Diseases, 10th Edition (ICD-10) codes likely to include myelitis.

Background: Total knee arthroplasty (TKA) is a surgical procedure that induces intense acute postoperative pain, but the mechanisms that amplify post-TKA pain remain incompletely understood. Endocannabinoids, such as 2-arachidonoylglycerol (2-AG), are endogenous lipids that can produce antinociceptive effects. However, hydrolysis of 2-AG by monoacylglycerol lipase (MAGL) generates arachidonic acid, the precursor to a host of eicosanoids that enhance pain.

Vitamin B12 is a vital water-soluble vitamin containing a central cobalt atom within its corrin ring structure. It exists in several derivatives, among which methylcobalamin (MeCbl) and adenosylcobalamin (AdCbl) are the biologically active forms that serve as cofactors in essential enzymatic reactions. Although the neurological and hematological consequences of vitamin B12 deficiency have been extensively studied, its role in immune regulation remains less well understood.

Background: Meningitis following cerebrospinal fluid (CSF) leak is associated with substantial morbidity and mortality. Current strategies for prevention, therapeutic options, and surgical timing lack consensus due to insufficient risk stratification tools. This study aimed to identify meningitis risk determinants and develop a predictive model to facilitate early detection in CSF leak patients.

Objective: This study aimed to identify distinct REM sleep characteristics that differentiate type 1 narcolepsy (NT1) from type 2 narcolepsy (NT2) using polysomnography (PSG), while acknowledging the need for future validation against other hypersomnia disorders.
Methods: A retrospective review included 31 patients with NT1, 21 patients with NT2, and 24 healthy participants. Each participant underwent overnight PSG and a subsequent multiple sleep latency test (MSLT) to assess REM sleep parameters including average REM density, neck myoclonus index, and leg movement index.
