Article Abstract

A 60-year-old female sustained a left unilateral C3-4 facet dislocation and underwent urgent open reduction and posterior instrumentation. The patient subsequently developed delayed onset right upper extremity weakness affecting the C5 myotome, prompting a diagnosis of post-operative right C5 palsy (C5P). With conservative management, the patient had a complete neurologic recovery in eight months. Surgeons must be aware of the development of post-operative C5P whenever operating on the cervical spine, regardless of the operative level or procedure performed. This is the first documented case of C5P after the reduction of a facet dislocation, which occurred after a procedure not involving the C4-5 level.

Molecular crystals are subjected to mechanical stress that can alter their slip planes which are structural features that play a critical role in governing mechanical behaviors such as milling, that requires fundamental insight for the successful manufacturing and performance of solid oral dosage forms. Milling is a critical unit operation in the manufacturing of pharmaceutical dosage forms. During this process, slip planes may become disrupted and dislocations can form, potentially inducing plastic deformation and significantly altering the physicochemical properties of active pharmaceutical ingredients (API).

Ponticulus posticus (PP), also known as arcuate foramen, is an anatomical variation of the atlas (C1), which is an ossification of the posterior atlanto-occipital membrane and through which the V3 segment of the vertebral artery (VA) runs. Placement of a polyaxial screw in the C1 lateral mass is considered risky in those with PP because of possible risk for VA injury. We report a case with bilateral PP in which posterior fixation for unstable upper cervical spine injury was performed.

Bilateral perched facets of the cervical spine are relatively common in trauma cases; however, similar injuries in the thoracic spine are exceedingly rare due to the inherent stability provided by the rib cage and associated ligamentous structures. We report a rare case of bilateral perched facets at the T10/11 level in a 17-year-old female gymnast who fell from a height of 3 m during training. She presented with severe back pain but no neurological deficits and was ambulatory on arrival.

Background: Varus posteromedial rotatory instability typically results in an isolated anteromedial coronoid (AMC) fracture without an associated radial head fracture or elbow dislocation. However, recent series have shown concomitant radial head fractures and elbow dislocations may be seen in association with AMC fractures in other injury patterns with an unclear effect on patient outcomes. Therefore, our objective was to compare postoperative patient outcomes in AMC fractures with and without concomitant radial head fractures or elbow dislocations.
