Objectives: Double-balloon enteroscopy (DBE) is effective for managing small bowel (SB) diseases. We aimed to evaluate the patient outcomes of DBE polypectomy in Peutz-Jeghers syndrome (PJS) with large SB polyps at surveillance imaging studies and to identify the risk factors for SB surgery.
Methods: Forty-five PJS patients who underwent regular SB surveillance imaging studies from 2005 to 2023 were retrospectively included. DBE was performed for polyps > 15 mm detected by imaging studies, and DBE polypectomy was conducted for those > 10 mm or symptomatic ones.
Results: Patients' average age at PJS diagnosis and surveillance initiation was 19.9 and 27.8 years, respectively. Thirty-one (68.9%) patients had laparotomy before surveillance. Each patient underwent 2.7 DBE procedures at a 31.0-month interval. An average of 7.8 and 4.4 polyps were removed during the first and second DBE procedures (p = 0.070). During 9 (8.2%) DBE procedures, complications, including two perforations requiring surgery, occurred. During the follow-up period, 11 patients required SB surgery, with a median time to surgery of 155 months. Patients with ≥ 5 polyps removed at initial DBE had a higher cumulative probability of SB surgery than those with < 5 polyps (hazard ratio [HR] 9.65, p = 0.031). Patients with ≥ 3 laparotomies before surveillance tended to have an increased surgery risk (HR 9.98, p = 0.078).
Conclusions: DBE polypectomy effectively manages large SB polyps detected by imaging surveillance in PJS over the long term. Early initiation of surveillance should be emphasized to minimize the risk of SB surgery.

Objectives: To balance the extended functional urinary voiding and morbidity outcomes amid Ileal W and Y-shaped contrasted to spherical ileocoecal (IC) orthotopic bladders subsequent prostate-sparing radical cystectomy (PRC) versus standard radical cystoprostatectomy (RC).
Material And Methods: Two hundred eight male bladder cancer patients were grouped into 98 RC followed by 43-W, 31-Y, and 23-IC in comparison to 110 PRC followed by 35-W, 37-Y, and 38-IC. The functional voiding outcomes were determined by detailed patients' interview and urodynamic studies (UDS).

Introduction: We investigated the associations between diabetes (type 2), hypertension and hypercholesterolemia with mild cognitive impairment (MCI) and Alzheimer's disease (AD) diagnoses by race-ethnicity and sex.
Methods: Data (n = 22,950) were derived via the National Alzheimer's Coordinating Center. Logistic regression was used to assess the relationship between each comorbid condition and MCI and AD.

Background And Objective: The apex-to-base transperineal (TP) prostate biopsy needle trajectory may better sample the anterior prostate than the transrectal approach. There is evidence that African American men are more likely to harbor anterior tumors, suggesting that TP biopsy would result in better detection in these men. We performed a secondary retrospective analysis of prospective randomized trial data to determine whether African American race is associated with anterior tumor location and better detection with TP biopsy.

Dome-shaped macula (DSM) is a distinctive anatomical entity characterized by an inward convexity of the macula, initially described in highly myopic eyes within posterior staphyloma, but it is now recognized as occurring across a broader spectrum of refractive conditions, including mild myopia and even emmetropia. Since its initial description in 2008, advances in imaging technologies and longitudinal studies have significantly improved our understanding of DSM. This review analyzed the recent literature, focusing on publications from the last 10 years.

Accurate classification of lung lesions at necropsy is crucial for guiding the diagnostic process and ensuring effective management of porcine respiratory diseases. Post-mortem inspection of the lungs during slaughter also provides valuable insights into disease occurrence, offering useful feedback on the efficacy of on-farm prevention and control strategies. However, manual assessment protocols may be impaired by high slaughtering speeds and low inter-rater agreement, which limits continuous data collection and hinders comparability.
