Ophthalmoplegic migraine was previously defined as migraine associated with oculomotor palsy. The condition has recently been termed recurrent painful ophthalmoplegic neuropathy (RPON). There is a paucity in the literature reporting the incidence and treatment strategies for RPON. This case report describes a 19-year-old male diagnosed with RPON and the management strategies implemented for treatment.

The extraocular muscles are integral to voluntary and reflexive eye movements, enabling precise gaze control through the coordinated action of six muscles. Variants in extraocular muscle anatomy, though rare, may contribute to disorders of ocular movement. We report a case of a 20-year-old female presenting with recurrent painful ophthalmoplegic neuropathy found to possess a rare anomalous muscular band between the superior and inferior rectus muscles identified on magnetic resonance imaging of the orbits.

Recurrent painful ophthalmoplegic neuropathy (RPON) is a rare neurologic disorder characterized by headaches and ocular cranial nerve paresis with no consensus on optimal treatment. While pharmacologic treatments are commonly used, limited research has explored nonpharmacologic options. A 36-year-old woman with a multi-year history of episodes of RPON lasting several months presented to a chiropractor with a week-long episode of headache and diplopia unresponsive to oral steroids and sumatriptan.

Recurrent painful ophthalmoplegic neuropathy (RPON) is a rare condition characterised by recurrent episodes of headache followed by ophthalmoplegia.An adolescent female presented with a throbbing unilateral headache associated with photophobia and phonophobia, progressing to binocular diplopia after 10-12 hours. She had two prior episodes that resolved spontaneously over 2-3 weeks.
