Article Abstract

We present a unique case of progressive neoaortic root and neoascending aortic dilatation, with poly-valvar regurgitation in a patient with Marfan syndrome and hypoplastic left heart syndrome palliated to Fontan procedure. This progressive dilation and valvar dysfunction necessitated a modified Bentall procedure and tricuspid valve repair. Our case highlights the impact of both a primary and secondary aortopathy complicating the Fontan procedure and the need for mechanical aortic valve replacement.

Study on the remodeling of distal residual dissection after surgery in patients with type A aortic dissection and Marfan syndrome.

Objective: To evaluate the remodeling of the distal aorta and outcomes after aortic surgery for type A aortic dissection (TAAD) in patients with Marfan syndrome and investigate whether morphologic characteristics of the dissection can predict negative remodeling.

Methods: Between 2013 and 2021, we performed total arch with a frozen elephant trunk for 325 patients with Marfan syndrome with DeBakey type I aortic dissection. Mean age was 47.

Mitral annular disjunction distance is associated with adverse outcomes in children and young adults with connective tissue disorders.

Department of Cardiology, Boston Children's Hospital, Boston, Massachusetts; Department of Pediatrics, Harvard Medical School, Boston, Massachusetts.

Introduction: Mitral annular disjunction (MAD) is a pathologic fibrous separation of the mitral valve hinge point from the ventricular myocardium. The aims of this study were to describe the range of MAD distance by cardiac magnetic resonance (CMR) in children and young adults with connective tissue disorders (CTDs) versus a healthy control sample, and to assess the MAD distance as a predictor of adverse cardiovascular outcomes.

Methods: This was a retrospective, single-center study of healthy subjects and patients with Marfan syndrome, Loeys-Dietz syndrome, Ehlers-Danlos syndrome, or nonspecific CTD who underwent CMR between 01/01/2000 and 01/01/2020.

Background: Fulminant myocarditis (FM) is a rare but serious inflammatory disease of the heart that should be considered for extracorporeal membrane oxygenation (ECMO) supportive therapy when it occurs. The diagnosis of FM is made more difficult in the context of Marfan's syndrome combined with aortic root dilation. We report a case of a patient on ECMO support and with comorbid Marfan's syndrome who was finally diagnosed with FM after computed tomography angiography (CTA) differentiated between FM, coronary artery disease, and aortic root dilation.

Marfan syndrome (MFS) is a hereditary connective tissue disorder that is primarily caused by mutations in the fibrillin-1 () gene. This disease predominantly affects the eyes, bones, and cardiovascular system, with cardiovascular complications posing the most significant threat to life. Currently, conventional treatments, which are based on pharmacological management and surgical interventions, aim to slow disease progression and manage life-threatening cardiovascular complications.

The David procedure has been extensively studied as an elective treatment when valve anatomy and function permit valve-sparing aortic root replacement. This approach is particularly beneficial in young patients who also require mitral valve repair and treatment for mitral annular disjunction. This video tutorial provides a step-by-step guide to the David V procedure in a female patient with Marfan syndrome, presenting with an aortic root aneurysm, severe mitral regurgitation and mitral annular disjunction.
