Article Abstract

Aim: To investigate the efficacy and mechanisms of indirect intense pulsed light (IPL) irradiation on meibomian gland dysfunction (MGD).

Methods: A total of 60 MGD patients was included in this prospective randomized controlled trial. Patients were randomly assigned 1:1 into two groups (3-mm group and 10-mm group) in which IPL was applied at distances from the lower eyelid margin of 3 and 10 mm, respectively. Both groups received three times treatment with 3-week interval. Meibomian gland yield secretion score (MGYSS), standard patient evaluation of eye dryness (SPEED) questionnaire, tear break-up time (TBUT), corneal fluorescein staining (CFS), and confocal microscopy were performed at baseline and after every treatment.

Results: After three IPL treatments, both groups had significant improvement in MGYSS (both <0.05). The non-inferiority test showed that improvement in 10-mm group was not inferior to that in 3-mm group (<0.001). In both groups, temporal regions of both upper and lower eyelids showed significant improvement in MGYSS. Scores of SPEED questionnaire in both groups declined significantly (both <0.001) and changes of SPEED had no difference between two groups (=0.57). Density of central corneal subepithelial nerves and TBUTs showed no statistically significant changes. The 3-mm group had improvement on corneal fluorescein staining (=0.048) and meibomian gland morphology (acini wall thickness =0.003, hyperreflective points =0.024) while the 10-mm group had not.

Conclusion: The efficacy of IPL indirect irradiation in improving meibomian gland secretion and alleviating dry eye symptoms remains unchanged with increase in treatment distance. IPL may primarily act on the functional improvement of the meibomian glands and corneal nerves.

Ocular surface and tear film in university students who use inhalant substances, mainly vapers.

Arch Soc Esp Oftalmol (Engl Ed)

Grupo de investigación GIESVI, Facultad de Optometría, Universidad Santo Tomás, Bucaramanga, Colombia.

Objective: To determine the state of ocular surface and tear film in college students who are users of inhalants, mainly vapers.

Methods: We conducted a descriptive, observational, and cross-sectional study with 62 participants whose sociodemographic characteristics, ocular surface status, tear film and variables related to vaping were evaluated. Individuals with a higher (5-7 times per week of consumption) and lower frequency of vaping (1-4 times per week of consumption) were evaluated.

Meibum - a lipid-rich secretion produced by holocrine Meibomian glands (MG) - plays a central role in maintaining ocular surface homeostasis. Previously, changes in MG lipidomes induced by inactivation of critical genes of meibogenesis, such as and others, were shown to cause MG-dysfunction- and dry eye-like signs in mice. Here, we describe the impact of the lipid composition of meibum on its physiological properties, specifically thermotropic/melting characteristics, using various wild type and mutant animals, and compare them with meibum of healthy humans and patients with abnormal meibum.

State Key Laboratory of Ophthalmology, Zhongshan Ophthalmic Center, Sun Yat-sen University, Guangdong Provincial Key Laboratory of Ophthalmology and Visual Science, Guangdong Provincial Clinical Research Center for Ocular Diseases, Guangzhou, 510060, People's Republic of China.

Objective: To explore alterations in tears and ocular surface throughout Descemet's Membrane Endothelial Keratoplasty (DMEK), thereby offering valuable insights for clinical intervention.

Methods: This study was conducted on 84 patients (84 eyes) with CED and received DMEK who visited the Zhongshan Ophthalmic Center of Sun Yat-sen University from April 2022 to January 2024. All patients were evaluated preoperatively and 1, 3, 6, and 12 months postoperatively, with 31 patients (31 eyes) post-24 months above.

Background: Dry eye disease (DED) represents a multifactorial condition characterized by ocular discomfort and visual disturbances. The management of DED relies heavily on accurate diagnosis to tailor effective treatments. Diagnostic approaches encompass both subjective and objective assessments.

Purpose: Sex hormone changes, often occurring during menopause, are implicated in meibomian gland dysfunction (MGD) and evaporative dry eye disease (DED). This study aimed to evaluate whether menopausal status affects the treatment outcomes of perfluorohexyloctane eye drops.

Methods: This post hoc analysis used data from a phase 3 trial (NCT05515471) of perfluorohexyloctane eye drops in patients with DED associated with MGD.
