Background: Coronoid fractures often occur with complex elbow dislocations, accounting for 15% of elbow fractures. The effectiveness of open reduction internal fixation (ORIF) versus arthroscopic-assisted reduction internal fixation (ARIF) surgeries for coronoid fractures remains uncertain. This study aimed to compare the outcomes of these two surgical approaches in treating varus posteromedial injuries.
Methods: This study was conducted and reported following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The study protocol was registered with PROSPERO. The primary outcomes assessed in this study were the functional outcomes measured by the Mayo Elbow Performance Score (MEPS) and the complications associated with each surgical approach.
Results: Analysis of the data from 759 patients included in this study revealed that the mean postoperative MEPS score was higher in the ARIF group compared to the ORIF group (97.5 Vs. 90.7), with the mean improvement in MEPS was 16.6 points (19 for ORIF and 14.3 for ARIF). The complication rate in the ORIF group was 24.6%, while the ARIF group reported a complication rate of 6%.
Conclusion: This study suggests that ARIF surgery can yield comparable and promising outcomes to ORIF surgery for coronoid fractures.
Level Of Evidence: IV.

An 8-year-old girl fell onto her outstretched arm, sustaining proximal ulna and radial neck fractures. After closed reduction and casting in the emergency department, radiographs showed improved alignment but limited bony detail. A CT scan performed 3 days later demonstrated 18° apex-medial angulation of the radial neck, slight radiocapitellar subluxation, and subtle calcification near the trochlear notch, concerning intra-articular injury.

Introduction: Complex proximal ulna fractures (including comminuted olecranon fractures and fracture-dislocations of the elbow) pose challenging management due to concomitant injuries to key stabilizers (radial head, coronoid process, and collateral ligaments). This study evaluates functional and radiographic outcomes following surgical treatment and explores the prognostic value of associated injuries, age stratification, and a new coronoid-centric classification system.
Methods: We retrospectively reviewed 43 patients with complex proximal ulna fractures treated between 2019 and 2024, with a minimum of 12 months follow-up (mean, 28 months).

Objective: Dislocations of the elbow are not common in skeletally immature patients. Herein, we present a case report on a rare pediatric posterior trans-olecranon fracture dislocation of the elbow, a type of dislocation that has never been reported in children. We aim to discuss the injury mechanism and introduce our treatment approach for this specific condition.

Background: Coronoid fractures significantly impact elbow stability, yet limited biomechanical data exists comparing fixation methods for different fracture types. This study aimed to compare the biomechanical performance of threaded K-wire versus cortical screw fixation in O'Driscoll type 2 and 3 coronoid fractures.
Methods: Twenty-eight synthetic ulnar bones were divided into four groups (n = 7 each): Type 2 with K-wire fixation, Type 2 with screw fixation, Type 3 with K-wire fixation, and Type 3 with screw fixation.

Background: Terrible triad of elbow injury (TTI) has great potential for joint instability with a relatively poor prognosis. Different surgical methods have been introduced for radial head fractures. In this study, we aimed to evaluate the results of treatment of radial head fractures with open reduction and internal fixation (ORIF) and resection in TTI patients when radius head replacement was not attainable.
