Article Abstract

The aim of this case report was to present an alternative therapeutic combination involving lip repositioning (LR) in the correction of gummy smile (GS). This treatment is less invasive than orthognathic surgery and is highly acceptable to patients, using a modified technique that combines myotomy with the insertion of polyester threads as a physical barrier against relapse.

The footplates of the medial crura are relevant structures that play an important role in the columella and nostril sill design. They also work as a connection between the nasal and oral muscles, as the insertion of the superficial orbicularis oris nasalis (SOON) is placed into the footplates and the continuation of the superficial muscular aponeurotic system (SMAS) is situated within the membranous septum. Moreover, the SOON is a powerful depressor of the nasal tip.

Aim: This study aimed to evaluate the accuracy of a personalized orthognathic surgical guide (POSG) system using customized titanium fixation plates versus 3D-printed splints for maxillary repositioning in Le Fort I osteotomy.

Trial Design: A randomized clinical trial with parallel-arm design and 1:1 allocation ratio with a superiority framework.

Methods: Twelve patients requiring Le Fort I osteotomy were randomly assigned to two groups based on the surgical transfer technique: a customized titanium plate group (study group) and a 3D-printed splint group (control group).

Objectives: To evaluate and compare the treatment of excessive gingival display (EGD) with lip repositioning surgery (LRS), with or without presurgical Botox injection, using relapse as the primary outcome.

Materials And Methods: Twenty-two periodontally healthy patients with EGD were recruited and allocated into two equal groups. The intervention group ( = 11) underwent LRS with Botox preinjection into the smile muscles 1-2 weeks before surgery, while the control group ( = 11) underwent only LRS.
