Cytotoxic lymphocyte pore-forming protein termed perforin-1 (PFP) produced by NK and CD8 T cells plays a critical role in viral and tumor-related immunity. Deleterious mutations in the gene encoding PFP lead to its dysfunction and impaired cytotoxic activity, causing hemophagocytic lymphohistiocytosis. Familial hemophagocytic lymphohistiocytosis type 2 is a rare, life-threatening, hyperinflammatory disorder that predominantly affects infants and children.

Sepsis poses a critical threat to global health, mainly due to the disruption of immune homeostasis, which critically influences both early death and long-term adverse outcomes. Current evidence shows that regulatory T (Treg) cells-key mediators of adaptive immunity-play an essential role in maintaining immunological balance during sepsis progression. During the initial hyperinflammatory phase, Treg cells actively suppress excessive inflammation, reducing tissue damage.

Objectives: Obesity is an important risk factor for atherosclerotic cardiovascular disease. This cardiovascular risk remains increased even after substantial weight loss by bariatric surgery. Innate immune cells are important regulators of atherogenesis and can adopt a long-term hyperinflammatory phenotype via epigenetic reprogramming, called "trained immunity".

Objective: Severe dengue in children may present with a hyperinflammatory phenotype resembling cytokine storm or secondary haemophagocytic lymphohistiocytosis, characterised by refractory shock, hepatic dysfunction and multiorgan failure. We evaluated the use of interleukin 1 receptor antagonist therapy (anakinra) in children with severe dengue.
Design: We conducted a retrospective study of children admitted to a tertiary paediatric intensive care unit in India between May 2021 and December 2023 with confirmed severe dengue who received anakinra.
