Drug-related pneumonitis (DRP) caused by epidermal growth factor receptor (EGFR)-tyrosine kinase inhibitors (TKIs) is a fatal adverse event in patients with EGFR-mutant non-small cell lung cancer (NSCLC). The diagnosis of DRP is based on radiological findings, the temporal association of presentation with the initiation of a systemic therapeutic agent, and the exclusion of other likely causes. Here we report a case in which severe adenoviral pneumonia mimicking DRP occurred during treatment with osimertinib, and osimertinib was successfully resumed after recovery from adenoviral pneumonia.

Red yeast rice (RYR), a commonly used supplement with statin-like properties, is generally considered safe but may cause severe adverse effects such as rhabdomyolysis. We report a rare case of severe RYR-induced rhabdomyolysis complicated by acute kidney injury (AKI) and respiratory failure, with diaphragmatic dysfunction as a key contributing factor. A 78-year-old woman developed progressive proximal muscle weakness, dyspnea, and tea-colored urine after taking RYR (2 g/day) for 1 month.

Rationale: Bilateral symmetrical globus pallidus (GP) involvement associated with coronavirus disease 2019 (COVID-19) infection is an extremely rare finding among cases of COVID-19-related encephalopathy.
Patient Concerns: A 76-year-old man was diagnosed with pneumonia and subsequently tested positive for COVID-19. He was transferred to the hospital on day 9 after symptom onset due to worsening respiratory distress and hypotension.

A 51-year-old man was referred to our hospital because of a worsening liver function after a follow-up of13 years for elevated hepatobiliary enzyme levels of unknown etiology. The IgG4 (4,990 mg/dL) levels were markedly elevated. Imaging revealed splenomegaly, intrahepatic bile duct dilatation, and an early enhancement area in the right lobe.

Human genetics supports a causal involvement of IL-6 signaling in atherosclerotic cardiovascular disease, prompting the clinical development of anti-IL-6 therapies. Genetic evidence has historically focused on IL6R missense variants, but emerging cardiovascular treatments target IL-6, not its receptor, questioning the translatability of genetic findings. Here we develop a genetic instrument for IL-6 signaling downregulation comprising IL6 locus variants that mimic the effects of the anti-IL-6 antibody ziltivekimab and use it to predict the effects of IL-6 inhibition on cardiometabolic and safety endpoints.
