Article Abstract

Background:  Neuroregulation of sexual functions requires coordination of parasympathetic, sympathetic, and somatosensory neuronal pathways. The nerves formed by the lower lumbar plexus provide the innervation of the urogenital organs. Lower lumbar disc hernias (LDHs) might impair the neuroregulation of sexual functions by compressing the neural structures. This prospective study aims to evaluate the effect of lower LDHs on libido and sexual dysfunctions.

Methods:  A total of 61 sexually active patients diagnosed with single-level LDH who underwent unilateral microdiscectomy were included in the study. The patients' healthy sex partners were included in study as controls. The International Sexual Function Index-5 (IIEF-5) and Female Sexual Function Index (FSFI) were used for sexual assessment of male and female participants, respectively. Visual Analog Scale (VAS) was used for pain assessment.

Results:  According to the FSFI scores, all 20 female patients had preoperative sexual dysfunction, which improved in 15 (75%) patients after surgery ( < 0.001). According to the IIEF-5 score for men, 38 of 41 male patients had preoperative sexual dysfunction, which improved in 26 (68.4%) patients postoperatively ( < 0.001).

Conclusion:  The preoperative sexual dysfunctions of LDH patients improved significantly after the microdiscectomy.

Tiredness may be associated with increased or decreased sexual experience and behavior while fatigue seems to have a predominantly negative effect, although evidence is scarce. This ecological momentary assessment study is the first to examine associations between tiredness or fatigue and concurrent / subsequent sexual desire or sexual arousal and previous / subsequent sexual activity in daily life, including event-based measurements and considering gender differences. Healthy heterosexual individuals (n = 63), aged between 19 and 32 years and in a relationship, indicated their tiredness, general fatigue, physical fatigue, sexual desire, and sexual arousal on an iPod seven times daily over 14 days, and any event-based occurrences of sexual activity.

Dendritic cells (DCs) are potent antigen-presenting cells and play a key role in facilitating the sexual transmission of HIV, functioning as a delivery system responsible for trafficking the virus from exposed barrier sites to their key target cells, CD4 T cells. Although the role of DCs in HIV transmission is well established, the recent advent of high-parameter, single-cell detection technologies, coupled with improved cell isolation techniques, has led to the rapid reclassification of the DC landscape, particularly within human barrier tissues. The identification of new subsets introduces the challenge of incorporating previously understood transmission principles with new, cell-specific, functional nuances to identify the key DCs responsible for facilitating HIV infection.
