Article Abstract

Background: There is limited data on the effectiveness of training interventions to improve the delivery of bad news.

Methods: This preliminary research included pre-post assessments and an open-ended survey to evaluate the effectiveness and perceived value of training on delivering bad news for 26 first- and second-year fellows from five adult and pediatric fellowship programs.

Results: There was a significant increase in faculty assessment scores (34.5 vs. 41.0, respectively, Z = -3.661, < 0.001) and Standardized Patient (SP) assessment scores (37.5 vs .44.5, respectively, Z = -2.244,  = 0.025). Fellows valued having a standard framework to aid in the delivery of bad news; receiving targeted feedback and having the opportunity to apply their skills in a subsequent case.

Conclusions: A one-hour, four-phase lesson plan that includes an individualized training approach and simulation do-overs can be effective and valuable for preparing fellows to deliver bad news.

Background: This study investigated the comparable efficacy of sustained low-efficiency dialysis (SLED) over continuous renal replacement therapy (CRRT) in maintaining hemodynamic stability in critically ill patients with acute kidney injury (AKI) in an intensive care unit (ICU).

Methods: A single-centre, prospective observational study was conducted at Jaslok Hospital and Research Centre, Mumbai, involving 67 patients requiring RRT in the ICU. Thirty-five patients were included in the CRRT (Baxter Prismaflex System, USA) cohort, and 32 patients were included in the SLED (Fresenius 4008S, Fresenius Medical Care, Bad Hamburg, Germany) cohort.

A 76-year-old man with effort angina was admitted to Cardiology Department for an elective coronary angiography (CA). Diagnostic CA revealed severely calcified three-vessel coronary artery disease (CAD) with chronic total occlusions (CTO) of the right dominant coronary artery (RCA) and of the left circumflex artery (LCX) ostium, and severe calcific stenosis of the left descending artery (LAD). An urgent percutaneous coronary intervention (PCI) to treat LAD was carried out because of ECG modifications and chest pain immediately after CA.

Latines in the United States experience considerable rates of depression and while behavioral activation is an effective treatment for depression, internal and external barriers contribute to the underutilization of behavioral health services among Spanish-speaking Latines (SSLs). Fortunately, internet-based interventions have the potential to mitigate these barriers given their increased privacy, cost-effectiveness, and logistical flexibility. The present (pilot) study examined clinical outcomes and feasibility (engagement, satisfaction) with a culturally attuned behavioral activation telenovela intervention (BAD-TI) for SSLs using a quasi-experimental design.
