Article Abstract

Objective: This study aimed to evaluate the feasibility of real-time visualization and mapping of the right phrenic nerve (RPN) by using intracardiac echocardiography (ICE) during atrial fibrillation (AF) ablation.

Background: RPN injury is a complication associated with the ablation of AF. Multiple approaches are currently being used to prevent and detect RPN injuries. However, none of these approaches can directly visualize the RPN in real-time during the ablation procedure.

Methods And Results: The RPN was detected using ICE. The RPN and its adjacent structures were analysed. The relationship between the RPN's distance from the superior vena cava (SVC) and its pacing capture threshold was quantified. The safety of SVC isolation guided by the ICE-visualized RPN was evaluated. Thirty-eight people were enrolled in this study. The RPN was visualized by ICE in 92% of patients. It ran through the space between the SVC and the mediastinal pleura and had a 'straw'-like appearance upon ICE imaging. The course of the RPN was close to the SVC (minimum 1.0 ± 0.4 mm) and the right superior pulmonary vein (minimum 14.1 ± 7.3 mm). There was a positive linear correlation between the RPN's capture threshold and its distance from the SVC (Spearman's correlation coefficient = 0.728, < 0.001). SVC isolation was guided by the RPN; none of the patients developed an RPN injury.

Conclusions: RPN can be visualized by ICE in most patients, thus providing a novel approach for the real-time detection of RPN during AF ablation.

Robotic partial nephrectomy (RPN) is a standard-of-care nephron-sparing intervention for clinical stage T1 renal masses (cT1RMs). We evaluated RPN outcomes performed using the da Vinci single port (SP) and multiport (Xi) platforms in a statewide quality improvement collaborative since SP introduction in Michigan. MUSIC-KIDNEY maintains a prospective registry of newly diagnosed cT1RM.

Objectives: To develop a proactive risk assessment model for human papillomavirus (HPV) genotyping testing by integrating Failure Mode and Effects Analysis (FMEA) with quality indicators (QIs), ensuring compliance with ISO 15189:2022 and improving diagnostic accuracy.

Methods: A multidisciplinary team designed and performed detailed FMEA across pre-analytical, analytical, and post-analytical phases of HPV genotyping testing. To improve objectivity, we integrated Sigma metrics into the FMEA framework through a molecular diagnostics-specific model of QIs (MQI).

Background: Papilledema and other optic neuropathies are critical findings in neuro-ophthalmology that require timely and accurate diagnosis. This study evaluates the performance of a deep learning system (DLS) to identify papilledema and other optic neuropathies, when applied to a large dataset of retinal photographs acquired prospectively with a handheld nonmydriatic camera in a neuro-ophthalmology department.

Methods: International multiethnic, multicenter study including 20,533 retinal photographs (10,647 patients) from 31 centers worldwide.

Drowning is a major, yet preventable, public health issue causing an estimated 300,000 deaths globally in 2021. This study assessed the current state of drowning prevention efforts in the Americas, examining the presence of governance structures, policies, legislation and the implementation of key interventions to reduce drowning risks. Data were collected from 26 countries in the region through a World Health Organization-led initiative using a structured questionnaire and national consultations.
