During pressure support ventilation (PSV), excessive assist results in weak inspiratory efforts and promotes diaphragm atrophy and delayed weaning. The aim of this study was to develop a classifier using a neural network to identify weak inspiratory efforts during PSV, based on the ventilator waveforms. Recordings of flow, airway, esophageal and gastric pressures from critically ill patients were used to create an annotated dataset, using data from 37 patients at 2-5 different levels of support, computing the inspiratory time and effort for every breath. The complete dataset was randomly split, and data from 22 patients (45,650 breaths) were used to develop the model. Using a One-Dimensional Convolutional Neural Network, a predictive model was developed to characterize the inspiratory effort of each breath as weak or not, using a threshold of 50 cmHO*s/min. The following results were produced by implementing the model on data from 15 different patients (31,343 breaths). The model predicted weak inspiratory efforts with a sensitivity of 88%, specificity of 72%, positive predictive value of 40%, and negative predictive value of 96%. These results provide a 'proof-of-concept' for the ability of such a neural-network based predictive model to facilitate the implementation of personalized assisted ventilation.

The natural background radiation to humans comes both from outside the Earth (cosmic radiation) and from radioactive isotopes found on Earth (terrestrial radiation). Radionuclides of terrestrial origin are partly external exposure and partly internal exposure via ingestion and inhalation. One of the most important sources of external terrestrial radiation is soil, where the most significant isotopes found are uranium-238, uranium-235 and thorium-232, members of the decay series, and potassium-40.

Background: PRAGMA-CF is a clinically validated visual chest CT scoring method, quantifying relevant components of structural airway damage in CF. We aimed to validate a newly developed AI-based automated PRAGMA-AI and Mucus Plugging algorithm using the visual PRAGMA-CF as reference.

Microplastics (MPs) and benzotriazole ultraviolet stabilizers (BUVs) are emerging contaminants in airborne PM, yet their co-occurrence patterns, seasonal dynamics, and health risks remain poorly understood. We conducted a year-long study (2023-2024) in Shijiazhuang, a northern Chinese city, analyzing PM samples (n = 117) for MPs (abundance, size, polymer type) and BUVs (UV-320, UV-327, UV-328) via microscopy, Raman spectroscopy, and UPLC-MS/MS. Correlations with criteria air pollutants (PM, PM, NO, CO and O) and meteorological factors (temperature, humidity and wind scale) were assessed using spearman analysis.

Sarcopenia affects 20%-40% of chronic obstructive pulmonary disease (COPD) patients, significantly reducing muscle strength and functional capacity, leading to a decline in quality of life. This study reviews the impact of sarcopenia in COPD and evaluates effective therapeutic strategies. Findings suggest that pulmonary rehabilitation, combined with aerobic and resistance exercises, and supplemented with protein and vitamin D, enhances muscle function and reduces the prevalence of sarcopenia.

Background: Dark-field radiography of the human chest has been demonstrated to have promising potential for the analysis of the lung microstructure and the diagnosis of respiratory diseases. However, most previous studies of dark-field chest radiographs evaluated the lung signal only in the inspiratory breathing state.
Purpose: Our work aims to add a new perspective to these previous assessments by locally comparing dark-field lung information between different respiratory states to explore new ways of functional lung imaging based on dark-field chest radiography.
