This study presents a rare case of severe acute bacterial skin and soft tissue infection (ABSSSI) following freshwater fish spike injury in a 73-year-old man. Within 24 hours of sustaining the wound, the patient developed septic shock and progressive necrotizing fasciitis. Despite early administration of broad-spectrum antibiotics and intensive care, his condition deteriorated, necessitating below-the-elbow amputation on hospital day four.

Background: Previous studies have suggested that daily symptoms are a marker of bronchiectasis disease activity and could therefore identify patients at increased risk of exacerbation. However, international bronchiectasis guidelines recommend long-term macrolide treatment only in patients with three or more exacerbations per year. We aimed to investigate if symptoms independently predict future exacerbations and therefore identify additional responders to long-term macrolide treatment.

Background: This study aims to design a cost-benefit Integrated and Repeated Public Health Surveillance System (IRPHS) to monitor key health indicators, identify emerging health concerns, and support community health interventions through the provision of timely and systematic data for informed policymaking.
Methods: This study introduced a novel framework for public health surveillance in Iran by integrating multidisciplinary indicators into a single, telephone-based platform. Using structured consensus across 32 expert panels, remote-friendly and policy-relevant indicators were identified and operationalized.

Antimicrobial resistance (AMR) disproportionately affects people who are immunocompromised due to their frequent encounters with the health-care system and repeated, prolonged exposure to antibiotics. AMR threatens to undermine continued advances in cancer care, haematopoietic cell transplantation, and solid organ transplantation by severely restricting therapeutic options. The convergence of several factors in the diagnostic evaluation of infection among individuals with immunocompromising conditions contributes to excess and inappropriate antibiotic use.
