Plasma cell disorders, such as multiple myeloma, can cause numerous derangements of hemostasis. In this case report, we present a life-threatening coagulopathy in a patient with progressing multiple myeloma in which the antibody-producing heparin-like activity is a free light chain. The patient's bleeding was successfully treated using protamine sulfate, which then allowed treatment of her plasma cell leukemia. In the literature, other authors have reported similar patients who have responded to protamine sulphate either in vitro or in vivo , providing further evidence for the role of protamine sulfate in the reversal of coagulopathy and resolution of bleeding diathesis. Standard treatments of transfusion with fresh frozen plasma and cryoprecipitate are likely to be ineffective in life-threatening bleeding related to this mechanism (heparin-like effect), and it is essential that treating physicians are aware of this potential mechanism of bleeding in their patients.

During spermatogenesis, majority of the histones are replaced by protamines to enable the compaction of the sperm chromatin for efficient delivery into the oocyte. Even after the gross histone replacement, some histones are retained in the sperm ranging from 1-15% across mammalian species. The post-translational modifications (PTMs), present on the retained histones of the sperm regulate the embryonic development, post fertilization, making them the suitable candidate for paternal epigenetic inheritance.

Chondroitin sulfate proteoglycans (CSPGs) are inhibitory molecules deposited in the extracellular matrix of lesions in multiple sclerosis (MS). CSPGs maintain ongoing inflammatory processes and prevent oligodendrocyte progenitor cell (OPC) differentiation, resulting in impaired remyelination and chronic pathology in MS. Here, we profile low-molecular weight protamine (LMWP) as a small, positively charged peptide that binds to the negatively charged inhibitory sections of CSPGs.

It has been more than a century since the protamines were found. Protamine continues to exist as a nucleo-protamine complex, similar to how the histone does. In sperm cells of vertebrates, protamine binds to DNA to produce compact chromatin.

Methodologies used to evaluate bull fertility often overlook sperm chromatin, a nuclear structure critical for reproductive success due to its DNA composition, which is tightly bound to nucleoproteins called protamines. In mammals, the ratio between Protamine 1 (PRM1) and Protamine 2 (PRM2) plays a pivotal role in male fertility, with imbalances linked to infertility in species such as humans and mice. While bull sperm chromatin was previously believed to contain only PRM1, recent findings have confirmed the presence of PRM2, prompting questions about the impact of the PRM1:PRM2 ratio on bull fertility.

The challenging management of coagulation in cardiothoracic surgery requires a multifaceted approach. The use of pharmacological interventions such as tranexamic acid, heparin, and aprotinin minimizes bleeding but increases the associated risks of renal impairment and seizures. However, aprotinin has been replaced by tranexamic acid for safety reasons.
