Thrombocytopenia is a common feature of antiphospholipid syndrome (APS) and rarely requires treatment. Here we present the case of a 71-year-old man hospitalized for severe immune thrombocytopenia (ITP) secondary to APS and concomitant SARS-CoV-2 infection. The patient was successfully treated with systemic corticosteroids, intravenous immunoglobulins, and plasma exchange (PEX). Few data are published on the use of plasma exchange in the treatment of thrombocytopenia in non-catastrophic APS. In the setting of acute infection when immunosuppressive therapies might be contraindicated, plasma exchange may be considered an effective therapeutic option. SARS-CoV-2 infection may be a trigger for a relapse of immune thrombocytopenia.

Longitudinal extensive transverse myelitis (LETM) is a rare adverse event after vaccination. We present a case of severe myelitis in a 76-year-old man with positive anti-recoverin antibodies that occurred one week after RSVPreF3 vaccination against respiratory syncytial virus (RSV). The patient presented with severe spastic paraparesis, urinary retention, postural tremor of the upper extremities, hypesthesia, severely impaired proprioception and vibration sense in the lower extremities, and tonic spasms of the lower extremities.

Liver transplantation remains the definitive treatment for end-stage liver disease, yet rejection of the transplanted organ poses a significant challenge to long-term graft survival. We present a case of a 47-year-old woman who underwent liver transplantation for primary sclerosing cholangitis. Following the procedure, the patient experienced a rare phenomenon of dual rejection, characterized by both acute cellular rejection and antibody-mediated rejection.

We describe a case of a 56-year-old male who developed severe, refractory hypotension after an intentional ingestion of clozapine and who became hemodynamically stable after one session of therapeutic plasma exchange (TPE). The patient, who presented after an ingestion of clozapine, was found to have altered mental status and hypotension in the emergency department. Escalating catecholamine vasoactive agents were necessary to maintain adequate hemodynamics.
