Background: The classic surgical technique of the 15-mm incision minimally invasive approach is not suitable for AO 23-B3 distal radius fractures (abbreviated B3). We have modified this technique for B3. This study aimed to investigate the efficacy of the modified 15-mm incision minimally invasive approach with the conventional ORIF approach in the treatment of B3.

Case: An 11-month-old male child presented with severe congenital thumb hypoplasia, syndactyly of the thumb and index finger, and radial longitudinal deficiency. A novel three-stage surgical approach was performed to achieve functionality while respecting familial preference for thumb preservation using sequential syndactyly release, structural reconstruction using bone graft, and muscle and tendon transfers.
Conclusion: The reconstructed thumb demonstrated early wound healing, stable alignment, and maintained a 45-degree palmar abduction even up to 5 years postoperatively.

Background: Transfer of intercostal nerves to the radial nerve branch innervating the long head of the triceps muscle for elbow extension is indicated in patients with traumatic brachial plexus palsy that is either the pan-plexus type or C5-C7 palsy with no triceps muscle function. The procedure aims to restore triceps muscle function through the use of the intercostal nerves, which are expendable nerves, as donors.
Description: The procedure is performed by first identifying the third to fifth intercostal nerves and coaptating them to the radial nerve branch innervating the long head of the triceps muscle.

Background: The posterior interosseous nerve (PIN) is at significant risk during proximal radius exposure. This study investigates the risk of PIN traction injury by comparing PIN strain between the Kocher and Kaplan approaches during simulated plate fixation of radial head fractures using upper limb cadaveric models.
Methods: Fourteen fresh-frozen upper extremity specimens were dissected with Kaplan (n=7) and Kocher (n=7) approaches, and the PIN was exposed at the anterior elbow with a separate incision.
