Pleural empyema of extra pulmonary origin is uncommon and empyema secondary to a fistula between the urinary tract and thorax is extremely rare. We report a case of nephropleural fistula causing massive pleural empyema in a 64-year-old woman with a long history of urological problems, including nephrolitiasis and urinary tract infection. She was admitted with sepsis, fever, chills, tachypnea, productive cough and pyuria. At clinical examination, breath sounds were reduced over the left hemithorax. CT revealed a fistulous connection from the upper left calyceal group and the pleural space. Drainage of thoracic and perinephric collection was carried out, but nephrectomy and pleural decortication were required due to haemopurulent urine and decreased hemoglobin levels during the hospitalization. This case demonstrates the unusual and prolonged evolution of an obstructive hydroureteronephrosis complicated by pyonephrosis, culminating in retroperitoneal abscess that fistulized into the pleural space, leading to empyema.

ObjectivesThe purpose of this retrospective, descriptive study was to assess CT findings and short-term outcome of cats with pyothorax presented to a referral hospital.MethodsThoracic CT studies of 41 cats with pyothorax comprising pre-contrast lung and soft tissue reconstructions and post-contrast soft tissue reconstructions were blindly reviewed by two European College of Veterinary Diagnostic Imaging (ECVDI) board-certified radiologists and a third-year ECVDI resident, referencing a predetermined list of imaging features. Clinical outcomes, including treatment options, survival to discharge and length of hospitalisation, were recorded.

Despite the initial treatment with intravenous ampicillin-sulbactam and chest tube drainage, the patient's condition did not improve. On day 12, Campylobacter coli resistant to both macrolides and fluoroquinolones was identified in the pleural fluid.

Serotype 3 pneumococcal infection can cause severe invasive disease, even in immunocompetent adults, and is potentially preventable by vaccination. Invasive pneumococcal disease caused by serotype 3 is currently the most frequent serotype found in adults in Denmark. In this report, we present a 62-year-old immunocompetent man diagnosed with a severe pleural empyema caused by serotype 3, requiring a long course of antibiotics.

Unlabelled: The recovery of microbial pathogens from sterile body fluids in children poses challenges, including the low sensitivity of conventional culture. Pre-treatment with empiric antimicrobials can render the pathogen non-viable. In such cases, and with fastidious organisms like , molecular methods are useful for identification of the causative agent.
