Article Abstract

Background: Awake fibreoptic nasotracheal intubation is an effective technique for the management of patients with difficult airways. Adequate sedation with effective topicalization of the airway is important to overcome discomfort and achieve intubation successfully.

Aim And Objectives: Our aim was to compare the effectiveness of dexmedetomidine-midazolam with fentanyl-midazolam infusion for providing conscious sedation during fibreoptic intubation in patients with anticipated difficult airway under topical anaesthesia.

Materials And Methods: Thirty adult patients of ASA physical status classification I and II with anticipated difficult airway and planned for elective awake nasal fibreoptic intubation under conscious sedation were randomly allocated into two groups. Dexmedetomidine 1 μg.kg-1 diluted in 50 ml saline was infused in Group DM over 10 min and Fentanyl 2 μg.kg-1 diluted in 50 ml saline was infused in Group FM over 10 min. Topicalization of the airway was done in all patients. All patients were assessed for sedation score, ease of endotracheal tube placement, patient comfort and cooperation, tolerance to endotracheal tube, any adverse events and recall of procedure.

Results: The score of the modified OAA/S was comparable between the groups ( > 0.05). Quality of AFOI was comparable in both groups ( > 0.05). The intubation time and first EtCO2 were significantly lower in dexmedetomidine group ( <0.05). Group DM also showed better hemodynamics and less episodes of desaturation than Group FM.

Conclusion: Fentanyl-midazolam and dexmedetomidine-midazolam are both effective for awake fiberoptic intubation under topical anesthesia. Dexmedetomidine allows better endurance and more stable hemodynamics.

Investigation of the small molecule-aptamer interaction is difficult, and it usually lacks information about the conformational change of aptamers that is important for their application. Here, we present the label-free investigation of small molecule-aptamer interactions using a modularized organic electrochemical transistor (OECT) platform. Leveraging the high sensitivity of the OECT, we measured the conformational change of the aptamer encountering its ligand.

Visual search relies on the ability to use information about the target in working memory to guide attention and make target-match decisions. The 'attentional' or 'target' template is thought to be encoded within an inferior frontal junction (IFJ)-visual attentional network. While this template typically contains veridical target features, behavioral studies have shown that target-associated information, such as statistically co-occurring object pairs, can also guide attention.

BACKGROUND Pregnancy-related anatomical and physiological changes, such as mucosal edema and increased oxygen demand, heighten risk of difficult airway, especially under general anesthesia. This study compared effects of spinal and general anesthesia on postoperative airway assessment tests in cesarean deliveries. We hypothesized upper airway changes can occur depending on anesthesia technique.

Tumor-immune interactions are central to cancer progression and treatment response, driving cell death through immune-mediated killing and resource-limited competition. In early-stage disease or following effective treatment, cancer populations are often small and difficult to observe directly. Disease monitoring therefore relies on the detection of biomarkers such as circulating tumor DNA (ctDNA) as noisy proxies to cancer size.

Background And Aims: Effective airway assessment prevents intubation failure. Despite the limited sensitivity of standard tools, ultrasonography (USG) offers promise, especially in predicting difficult laryngoscopies. Our study aims to evaluate tongue-to-oral height ratio (TTOHR) and anterior neck soft tissue measurements on airway USG to predict difficult laryngoscopy.
