Cardiovascular diseases are the leading cause of death. Clinical data used to decide treatment are hard to integrate and interpret, making optimal treatment selection difficult. Personalised models can be used to integrate clinical data into a physics and physiology-constrained framework, but their clinical application faces limitations due to complex calibration and validation.

Background: Amniotic fluid embolism is a rare potential complication in the peripartum period.
Case Summary: A 27-year-old nulliparous woman presented for elective induction at 41 weeks' gestation. Due to nonreassuring fetal heart tracing, the patient underwent emergent caesarean section complicated by pulseless electrical activity arrest secondary to suspected amniotic fluid embolism after fetus delivery.

Cardiogenic shock (CS) is a complex, multifactorial syndrome with substantial morbidity and mortality. Despite neutral results from randomized controlled trials, venoarterial extracorporeal membrane oxygenation (VA-ECMO) may be reasonable for select patients with refractory CS. A 37-year-old woman with morbid obesity and dilated nonischemic cardiomyopathy presented with Society of Cardiovascular Angiography and Intervention stage D biventricular CS, requiring vasopressor support and emergent escalation to ECPELLA, the combination of VA-ECMO and Impella (Abiomed), as a bridge to decision.
