Ga-DOTATATE positron emission tomography/computed tomography (PET/CT) has shown superiority over Indium-octreotide scanning for the detection of phosphaturic mesenchymal tumors (PMTs). We report a case of tumor-induced osteomalacia resulting from PMT which, although initially clinically suspected, was not localized on octreotide scintigraphy performed several years prior. Subsequent surgical excision of a presumed benign osseous lesion a few years later revealed the diagnosis on pathology. Imaging assessment using Ga-DOTATATE PET/CT following recent clinical suspicion for recurrence revealed an intense tracer-avid lesion at the primary tumor site. DOTATATE imaging plays an important role in localizing tumors with high somatostatin receptor expression, such as neuroendocrine tumors (pheochromocytoma, paraganglioma, and neuroblastoma), meningioma, and mesenchymal tumors, causing oncogenic osteomalacia.

Paraneoplastic syndromes (PNSs) are pathologic conditions produced by neoplasms not attributable to tumor invasion or metastasis. The clinical manifestations of PNSs can precede the diagnosis; these symptoms may serve as early indicators of underlying malignancy. Standard imaging modalities, such as computed tomography (CT) and magnetic resonance imaging (MRI), have limited sensitivity in detecting small or early-stage PNS-associated tumors.

Purpose: Lu-DOTA(0)-Tyr(3)-octreotate (Lu-DOTATATE) is a somatostatin receptor (SSTR)-targeting radiopharmaceutical that shows promise for treating metastatic pheochromocytomas/paragangliomas (PPGLs), a rare SSTR-expressing tumor.
Methods: In the first stage of this two-stage Simon phase II trial, 36 PPGL patients with RECIST 1.1 progression within 12 months were prospectively recruited into two genetic cohorts (succinate dehydrogenase []-mutated apparent sporadic, 18 per cohort) and treated with four cycles of Lu-DOTATATE.

A 56-year-old male patient underwent total thyroidectomy, and pathology revealed multicentric papillary thyroid cancer. His post-operative stimulated thyroglobulin value was >500 ng/mL. F-fluorodeoxyglucose positron emission tomography (PET) computed tomography revealed hypermetabolic metastatic pulmonary nodules, cervical, and mediastinal lymph nodes.

Background: Malignant pheochromocytoma and paraganglioma (mPPGL) are not so common and the factors predicting the metastatic behavior are not well understood.
Methods: This retrospective cohort study included 218 patients with PPGL managed at our center over a period of 11 years (2013-2024). The clinical profile and treatment outcomes of patients with mPPGL were studied and were compared with non-metastatic PPGL(non-mPPGL).

Purpose: To assess and compare the diagnostic performance of Gallium-68 DOTA-Tyr-octreotate positron emission tomography/computed tomography (Ga-DOTATATE PET/CT) and Iodine-123 metaiodobenzylguanidine single photon emission computed tomography/computed tomography (I-MIBG SPECT/CT) in detecting central nervous system (CNS) metastases of neuroblastoma (NB) and ganglioneuroblastoma (GNB), as well as to evaluate their impact on therapeutic management.
Methods: This prospective study enrolled 41 patients with NB and GNB who were suspected of having CNS metastases. All patients underwent paired I-MIBG SPECT/CT and Ga-DOTATATE PET/CT imaging within 1 week (median: 3 days; range: 1-7 days).
