Article Abstract

Objective: To evaluate the efficacy of docetaxel and androgen receptor axis-targeted (ARAT) agents in patients with castration-resistant prostate cancer (CRPC) with intraductal carcinoma of the prostate (IDC-P) using a propensity score-matched analysis.

Patients And Methods: We retrospectively identified 309 patients with CRPC from February 2007 to February 2016 at Nagoya University and its affiliated hospitals. All patients received initial androgen-deprivation therapy (ADT). After progression to CRPC, they received docetaxel or ARAT (abiraterone or enzalutamide) as first-line life-prolonging therapy. Docetaxel (70-75 mg/m ) every 3 weeks vs enzalutamide (160 mg) once daily orally or abiraterone (1 g) once daily plus prednisone (5 mg) twice daily orally was administered. The primary outcome of interest was overall survival (OS) from the time of CRPC diagnosis. A propensity score analysis with a 1:1 ratio using an optimal matching algorithm was used to adjust for confounding factors.

Results: Overall, 234 patients were analysed. Propensity score-matching identified 85 patients in each group. There were no significant differences in patient characteristics between the groups. The median OS in the docetaxel group was 38.2 vs 58.3 months in the ARAT group (P = 0.03). For patients with IDC-P, OS was significantly longer in the ARAT group than the docetaxel group (P = 0.01), and there was no significant difference in each group, as in patients without IDC-P (P = 0.67). A multivariate analysis showed that the presence of IDC-P, duration of primary ADT, visceral metastasis, and administration of ARAT as the first-line treatment for CRPC were independent prognostic factors for OS.

Conclusion: Administration of ARAT as the first-line treatment for CRPC may prolong OS more than that of docetaxel, especially in patients with IDC-P.

Purpose: Prostate-specific membrane antigen positron emission tomography (PSMA PET) is increasingly used to diagnose and stage prostate cancer. A PRIMARY score uses anatomical localization and uptake patterns to improve diagnostic accuracy. We evaluated the histopathology of patients with no uptake pattern (PRIMARY score 1) and the prevalence of intraductal carcinoma of the prostate (IDC-P) in this subset compared with those with an uptake pattern (PRIMARY score ≥ 2).

Candidemia Treatment is Improved by Infectious Disease Consultation: A Population-Based Cohort Study.

Introduction: Candidemia is a common bloodstream infection with morbidity and mortality. Both the European Society of Clinical Microbiology and Infectious Diseases and Infectious Diseases Society of America have evidence-based recommendations for its management. While infectious diseases consultation (IDC) reduced mortality, effects on guideline concordance remain unclear.

The clinical significance of 'atypical intraductal proliferation' (AIP) is uncertain when found in prostate needle biopsy without intraductal carcinoma (IDC-P) or intermediate/high-grade prostate carcinoma (PCa). A retrospective review identified 168 patients diagnosed with AIP. Twenty-five (15 %) were AIP alone, the rest with PCa.

Background And Objective: Accurate detection of unconventional histologies (UH) in prostate cancer (PCa) is crucial for treatment planning and prognosis. This systematic review and meta-analysis aimed to evaluate the accuracy of multiparametric magnetic resonance imaging (mpMRI) in detecting UH on prostatectomy, particularly cribriform architecture (CA) and intraductal carcinoma (IDC-P), in patients with localized PCa.

Methods: A literature search was conducted in major databases for studies published after 2000.

Purpose: The objective of this research is to assess the clinical importance of pleomorphic lobular carcinoma (PLC) by contrasting its survival outcomes and clinicopathological characteristics with those of classical invasive lobular carcinoma (ILC) and invasive ductal carcinoma (IDC).

Methods: Data from the SEER (Surveillance, Epidemiology, and End Results) database, which covers patients with breast cancer diagnosed between 2010 and 2021, are used in this retrospective analysis. Clinical, pathological, and demographic factors were noted.
