Article Abstract

Background: Demand for pancreatic surgery is rising, occasionally necessitating consecutive PDs to be performed by a single surgeon in the same workday. The safety of this practice is unknown.

Methods: Institutional prospective ACS-NSQIP data were reviewed for PDs (2013-2017). Instances where a single surgeon performed two PDs in the same day were a PD pair (PD1, PD2) and compared with univariable analysis. Paired vs. unpaired-PD matched analyses were performed.

Results: 661 PDs (25-PD pairs) were performed. PD1 and PD2 revealed similar infectious (12% vs16%), pulmonary (8% vs8%), cardiovascular (12% vs4%), and aggregate (24% vs24%) morbidity (P>0.05). Pancreatic fistula (B + C 0%), delayed gastric emptying (4% vs12%), hospital stay (9.3 vs8.8 days), and 30-day mortality (4% vs4%) were similar (P > 0.05). Matched outcomes were similar except higher cardiovascular morbidity for paired vs. unpaired PD (7% vs0%; P = 0.015).

Conclusion: With proper patient selection, and in experienced hands at high-volume centers, two consecutive open PDs may be safely performed.

Major ABO-Incompatible Platelet Transfusions Are Associated With Brain Ischemia After Intracerebral Hemorrhage.

Background: Major ABO-incompatible platelet transfusions are associated with poor intracerebral hemorrhage (ICH) outcomes, yet drivers for this relationship remain unclear. Brain magnetic resonance imaging (MRI) ischemic lesions after ICH are neuroimaging biomarkers of secondary brain injury and are associated with poor outcomes. Given that ABO-incompatible platelet transfusions can induce immune complex formation, thrombo-inflammation, and endothelial barrier disruption, factors that could exacerbate cerebral ischemia, we explored whether major ABO-incompatible platelet transfusions are risk factors for ischemic lesions on brain MRI after ICH.

Flexible suction-coagulation probe restores dexterity in robot-assisted surgery: bench-to-bedside evaluation.

Objective: Rigid suction-coagulation probes constrain the wrist-like articulation that is central to robotic surgery. We therefore designed a 5-mm single-use flexible suction ball coagulator (flex-SBC) with a modified core design to restore dexterity and assessed its mechanical performance and early clinical feasibility, including the effect of the common robotic gripping strategies on suction flow.

Methods: Preclinical.

Purpose: Sex differences in Hirschsprung disease (HSCR) incidence have been well documented, yet little is known about whether the effect of sex on postoperative outcomes. This study aims to investigate the sex differences of postoperative outcomes of HSCR using propensity score matching (PSM) analysis.

Methods: Retrospective review of 304 patients with HSCR who received single-stage laparoscopic transanal pull-through modified Swenson procedure in a single-center was conducted with assessments of clinical data.

Objective: Failure of arteriovenous fistula (AVF) maturation results in unnecessary patient risk and resource waste. We sought to identify potentially addressable risk factors for fistula failure-to-mature (FTM) to cannulation readiness.

Methods: We conducted a single-institution retrospective review of patients undergoing primary AVF creation from 2015-2021.

Purpose: This study aims to evaluate the efficacy of nail dynamization in patients with delayed union and nonunion of femur and tibia shaft fractures following intramedullary nailing, and systematically analyze the associated factors to guide surgeons.

Methods: A comprehensive search of PubMed, EMBASE, and Cochrane Library databases was conducted to identify relevant studies. We screened the literature based on the eligibility criteria, extracted relevant data, and assessed the quality of the included studies.
