Recurrence of endometriosis after conservative surgery is not an uncommon finding. There is no uniformity, however, on what the term 'recurrence' means. Recurrence is variously defined in the literature as the relapse of pain, clinical or instrumental detection of an endometriotic lesion, repeat rise in CA 125 levels, or evidence of recurrence found during repeat surgery. Consequently, the reported recurrence rate varies widely (0-89%) in the different series, depending on its definition and the type of study performed. As endometriosis recurrence seems to be an indeterminate enemy, we set out to examine exactly what we were fighting in our everyday battle. In this narrative review, we aimed to seek an answer to questions related to endometriosis recurrence, some of which are often asked by our patients.

Objective: Exosomes are secreted by most cell types and reflect the internal state of their cells of origin, playing crucial roles in the progression of various pathological conditions. Endometriosis is a chronic, estrogen-dependent inflammatory disease characterized by the ectopic presence of endometrial-like tissue outside the uterus, including in the ovaries, fallopian tubes, and peritoneal cavity. It primarily affects women of reproductive age and is often associated with infertility.

This meta-analysis assesses the impact of non-medical endometrioma treatments on pregnancy rates in infertile women with endometriosis. Based on a search of PubMed, Scopus, EMBASE and Web of Science, 24 prospective English-language studies (2000-2022, participant age 15-45 years, n = 2458) were identified that reported treatment and pregnancy rates based on the 2020 PRISMA checklist. Random effects modelling minimized bias; heterogeneity (I²) and publication bias were assessed using MedCalc.

Objective: To evaluate the safety, adhesion rates, and perioperative outcomes of monopolar and bipolar electrosurgical hysteroscopic metroplasty in women with a septate uterus.
Methods: We conducted a single-center retrospective analysis of 155 consecutive patients who underwent same-session laparoscopy and hysteroscopic septum resection between January 2021 and January 2025. Procedures were performed under surgeon discretion using either a monopolar loop with glycine distension or a bipolar loop with isotonic Ringer's lactate.

Decidualization is the transformation of endometrial stromal cells into functionally specialized cells during the early stages of pregnancy. Occurring in mammals that develop invasive hemochorial placentae, decidualization is a pivotal evolutionary adaptation in mammals that supports pregnancy establishment, implantation, and placentation in a limited number of animal species. During decidualization, an endometrial stromal cell undergoes profound genetic, epigenetic, and proteomic changes, allowing it to prevent immunological rejection and fostering the development of a newly implanted embryo.
