Purpose: We report the results and experience of a tertiary care center in laparoscopic transperitoneal heminephrectomy for the treatment of a non-functioning upper pole moiety of duplex kidney in adults.
Materials And Methods: The key point of the technique included the placement of a 6-Fr ureteric catheter in the healthy ureter at the beginning of the procedure under fluoroscopic guidance cystoscopically. A standard laparoscopic 3- to 4-port placement was done after placing the patient in a 45 to 90 degrees lateral decubitus position. The upper pole was mobilized transperitoneally and transected using a harmonic scalpel.
Results: A total of 17 patients aged 19 to 44 years underwent laparoscopic upper pole heminephrectomy. All patients had a complete duplicated renal collecting system on the ipsilateral side. Three patients were found to have ureterocele. The average blood loss was minimal (50-150 mL) with a mean of 95 ml. None of the patients required any blood transfusions. The mean operative time was 220 minutes (range, 160-315 minutes). The average length of hospital stay was 3 days (range, 2-4 days). Only 2 patients had Clavien-Dindo grade 3a complications, which were managed by double J stenting and percutaneous aspiration, respectively.
Conclusions: Laparoscopic heminephrectomy is playing a cornerstone role in the treatment of the non-functioning moiety of duplex kidneys. The procedure is safe, efficient and offers the typical preoperative and postoperative benefits of laparoscopic surgery. The rate of complications in adults is acceptable and is similar to that reported in pediatric patients.

Background: Adult duplex kidney with ureterocele is rarely diagnosed, especially when the upper moiety retains function. While pediatric laser decompression is established, the use of thulium fiber laser (TFL) with dual DJ stenting in adult duplex ureteroceles remains rare.
Case Presentation: A 23-year-old female presented in a tertiary care hospital with intermittent right flank pain and recurrent UTIs for 3 months.

To orchestrate ribosomal peptide synthesis, transfer RNAs (tRNAs) must be aminoacylated, with activated amino acids, at their 2',3'-diol moiety, and so the selective aminoacylation of RNA in water is a key challenge that must be resolved to explain the origin of protein biosynthesis. So far, there have been no chemical methods to effectively and selectively aminoacylate RNA-2',3'-diols with the breadth of proteinogenic amino acids in water. Here we demonstrate that (biological) aminoacyl-thiols (1) react selectively with RNA diols over amine nucleophiles, promoting aminoacylation over adventitious (non-coded) peptide bond formation.

Background And Objective: Persistent urinary dribbling in girls with duplex renal systems and ectopic ureters originating from poorly functioning renal moieties represents a complex clinical problem in pediatric urology. The incontinence associated with these anomalies is socially distressing and often refractory to conservative management. Traditional surgical options, including upper pole heminephrectomy or ureteroureterostomy, are effective but may be associated with significant operative morbidity, particularly when renal function is negligible and surgical access is challenging.

A C-nucleoside analog having pyren-1-ylpyridine as the base moiety has been synthesized and incorporated in the middle of a short oligodeoxynucleotide. A portion of this oligonucleotide is cyclopalladated at the modified residue, and the potential of both the metal-free and the palladacyclic oligonucleotide as hybridization probes for single-nucleotide polymorphism genotyping is assessed by melting studies on relevant duplexes using various techniques. Conventional ultraviolet (UV) melting profiles at 260 nm reveal considerable destabilization of the palladacyclic duplexes relative to their metal-free counterparts.
