The etiology of moyamoya disease (MMD) remains unknown. The main pathological finding is fibrocellular thickening of the intima, irregular undulation of the internal elastic lamina affecting the distal portions of the internal carotid artery and A1 and M1 segments. Our aim is to describe the histological and electron microscope ultrastructural characteristics of cortical MMD vessels (middle cerebral artery) in hemorrhagic and ischemic presentation along different Suzuki stages.

Background: Superficial temporal artery (STA)-middle cerebral artery (MCA) side-to-side microvascular anastomosis can achieve the same clinical effects as traditional STA-MCA end-to-side anastomosis in extracranial-intracranial revascularization surgery, furthermore, STA-MCA side-to-side anastomosis has the lower risk of postoperative cerebral hyperperfusion syndrome (CHS) and the potential to recruit all scalp arteries as the donor sources via self-regulation. Therefore, STA-MCA side-to-side microvascular anastomosis seems to be a revascularization strategy superior to traditional STA-MCA end-to-side anastomosis. In this study, we presented seven cases in which a STA-MCA side-to-side microvascular anastomosis was performed with a 4-5 mm long arteriotomy using the in-situ intraluminal suturing technique.

A 2-day-old neonate was referred to the vascular surgery service owing to concerns of limb perfusion after entrapment of a microcatheter in the middle cerebral artery. The catheter was inserted via the umbilical artery to treat a vein of Galen arteriovenous malformation. This catheter inadvertently became entrapped owing to device failure, was cut at the insertion site, and left in situ for 3 months.

This case report presents a clinical case of management of a patient with concomitant ischemic stroke and acute arterial ischemia of the right upper limb. Emergency thrombaspiration from the middle cerebral artery improved the patient's neurological status. A hybrid intervention was carried out to restore blood flow in the right upper limb: brachial artery arteriotomy with thrombectomy through an open approach combined with endovascular recanalisation and balloon angioplasty of the forearm arteries.

Object: Cerebral revascularization is an effective measure for dealing with complicated intracranial aneurysms and ischemic cerebro-vascular disease. Intra-operative thrombosis causing bypass occlusion is a severe issue that cause devastating consequences for complication in revascularization. We report our experiences regarding salvage maneuvers for intraoperative thrombosis in cerebral revascularization procedures and discuss the characteristics and culprits.
