Reconstruction of large, full-thickness palmar and digital defects is challenging. The glabrous skin of the palm provides excellent color and texture match for palmar hand and digital defects. The reverse thenar perforator flap, which was previously infrequently used, provides a method for primary closure of large glabrous palmar skin defects. Because of the origin of the source vessels, the reverse thenar perforator flap is a good choice for larger radial-sided palmar and finger defects. This flap provides good aesthetic results for both the donor and the recipient.

Heart failure with preserved ejection fraction (EF) and left ventricular (LV) hypertrophy (LVH) are typical phenotypes of transthyretin cardiac amyloidosis (ATTR-CA). Extracardiac manifestations, such as carpal tunnel syndrome, lumbar spinal stenosis, or biceps tendon rupture, frequently precede the development of overt cardiac involvement. We report a rare case of ATTR-CA presenting as heart failure with improved EF (HFimpEF) without LVH in an elderly patient, where the diagnosis was prompted by bilateral carpal tunnel syndrome, lumbar spinal canal stenosis, and persistently elevated troponin levels.

Background: Management of the extensive soft tissue injuries remains a significant challenge in orthopedic and plastic reconstructive surgery. Since the thumb is responsible for 40% of the functions of the hand, saving and reconstructing a mangled thumb is essential for the patient's future.
Case Presentation: This case report describes the management of a severe occupational thumb injury in a 25-year-old white Persian male who sustained an occupational injury to his left thumb, resulting in extensive burn, crush injury to the distal and proximal phalanx, and severe soft tissue damage to the first metacarpal, thenar, and palmar areas.

Introduction: The purpose of this study is to present a novel method of reconstructing oral and maxillofacial defects using the Radial Artery Retrograde Proximal Forearm Flap (RARPFF) and to evaluate the efficacy and safety of the forearm flap for direct closure of the donor site and its effect on hand function in the donor area.
Methods: A prospective study of 30 patients who underwent oral and maxillofacial reconstruction with RARPFF between June 2021 and April 2024 was conducted to evaluate the safety and efficacy of the flap and its influence on hand function in the donor area.

Background: Although palmar and digital soft-tissue resurfacing with intrinsic flaps may provide functional and aesthetic reconstruction, the donor site may result in a tight closure or need for a skin graft once the flap is raised. In this series, the "mini-shaped kiss flap" was used to increase the resurfacing efficiency and preserve hand function.
Methods: The mini-shaped kiss flaps, based on common palmar digital arteries (eg, princeps pollicis artery and ulnar palmar digital artery), were developed for the resurfacing of distal or proximal finger areas.
