Objectives: To assess frequency of gallbladder polyposis and carcinoma in metachromatic leukodystrophy (MLD).
Methods: We evaluated 34 patients with MLD (average age 16.7 years, age range 2-39 years) screened for gallbladder abnormalities by ultrasound. In the case of cholecystectomy, findings at pathology were reviewed.
Results: Only 8 of 34 patients (23%) had a normal gallbladder at ultrasound. Gallbladder polyps were visible in 8 patients (23%). Cholecystectomy was performed in 11 patients (32%). In these, pathology revealed various abnormalities, including hyperplastic polyps, intestinal metaplasia, prominent Rokitansky-Aschoff sinuses, and sulfatide storage.
Conclusions: Our results demonstrate that gallbladder involvement is the rule rather than the exception in MLD. The high prevalence of hyperplastic polyps, a known precancerous condition, and one death from gallbladder carcinoma at a young age suggest that MLD predisposes to neoplastic gallbladder abnormalities. As novel therapies for this patient group are emerging leading to increased life expectancy, we recommend screening for gallbladder abnormalities by ultrasound in order to prevent early death.

A cholecystopleural fistula is an extremely rare condition characterized by abnormal communication between the gallbladder and pleural cavity. This abnormal connection can lead to the leakage of bile or infectious material into the pleural space, potentially causing infected biliothorax. Here, we report a case of recurrent infected biliothorax caused by a cholecystopleural fistula in a 77-year-old man.

Background: Situs inversus totalis is an uncommon congenital condition characterized by the complete reversal of internal organ placements from their usual orientation. This syndrome presents challenges in both diagnostic and surgical care owing to the anatomical reversal, with an incidence rate of around 1 in 5000 to 20,000 infants.
Case Presentation: A 50-year-old Asian Sindhi Ayan female patient with well-managed hypertension presented with intermittent cramp-like discomfort in the upper left region of her abdomen, which exacerbated after consuming fatty foods, accompanied by feelings of nausea and vomiting.

Gallbladder volvulus is a rare but serious diagnosis that needs urgent surgical treatment to avoid morbidity and mortality related to gallbladder ischemia and should be suspected in elderly female patients with acute right upper quadrant pain with imaging features including enlarged, abnormally positioned gallbladder and specific key findings on CT including "whirl sign" representing twisting of the cystic pedicle and "beak sign" at the gallbladder neck.

Background And Objective: Gallbladder diseases present a critical challenge and can cause serious complications, if not diagnosed and treated promptly. Diseases including gallstone, inflammation, and other abnormalities may lead to a number of significant consequences, such as bile duct obstructions, chronic pain, infections, and in severe cases, life-threatening sepsis or gallbladder cancer.
Methods: This study presents a novel deep learning-based diagnostic model using an attention-guided residual convolutional neural network to classify nine distinct gallbladder diseases, including gallstones, abdomen and retroperitoneal pathology, cholecystitis, membranous and gangrenous cholecystitis, perforation, polyps and cholesterol crystals, adenomyomatosis, carcinoma, and various causes of gallbladder wall thickening.

Anatomical variations of both the vascular structures of the liver and gallbladder as well as the bile ducts, are very common. Detailed knowledge of the different courses of these vessels and ducts as well as their topographical relationships to one another, is essential in hepatobiliary procedures and surgery involving the upper gastrointestinal tract. Whenever possible, these variations should be identified preoperatively.
