Cognitive impairment and dementia, including Alzheimer's disease (AD), pose a global health crisis, necessitating non-invasive biomarkers for early detection. This review highlights the retina, an accessible extension of the central nervous system (CNS), as a window to cerebral pathology through structural, functional, and molecular alterations. By synthesizing interdisciplinary evidence, we identify retinal biomarkers as promising tools for early diagnosis and risk stratification.

Introduction: Differentiating acute tubular necrosis (ATN) from rejection in pediatric kidney transplant (KT) recipients remains challenging and necessitates invasive biopsy. Doppler ultrasound-derived resistive index (RI) is a noninvasive modality to assess graft status, but its diagnostic utility in children is unclear. This study evaluates RI's ability to distinguish ATN and rejection in KT.

Background: Risk stratification in posterior circulation ischemic stroke (PCIS) is challenging. Although the Posterior Circulation Ischemic Stroke Outcome Score (PCISOS) was developed to address this, its utility in minor PCIS and in identifying homogeneous populations for clinical trials or treatment-responsive subgroups remains uncertain.
Methods: CHANCE-2 (Clopidogrel in High-Risk Patients With Acute Non-disabling Cerebrovascular Events-II) was a multicenter, randomized trial that enrolled patients with minor stroke or high-risk transient ischemic attack who carried CYP2C19 loss-of-function alleles.

People with severe mental illness die 10-20 years earlier than the general population, mostly from preventable physical diseases. Fragmented care, under-screening and undertreatment of cardiovascular, respiratory, infectious and cancer conditions widen this gap. Embedding physical screening, proactive treatment, smoking cessation, cancer checks and multidisciplinary, person-centred care into psychiatric services could close this mortality divide.
