While intravenous administration of tissue plasminogen activator (tPA) remains the only FDA-approved treatment modality for acute ischemic stroke, many patients do not meet the criteria for intravenous tPA and are offered intra-arterial therapy. Rapid advances in devices and approaches have marked the evolution of thrombectomy over the past decade from rudimentary mechanical disruption, followed by intra-arterial thrombolytic infusions to increasingly effective thrombectomy devices. We review the critical advancements in thrombectomy technique that have evolved and the key anatomic and technical challenges they address, from first-generation Merci retrieval systems to second-generation Penumbra aspiration systems and third-generation stent retrievers, as well as nuances of their uses to maximize their effectiveness. We also highlight more recent advances that offer patients hope for more expedient vessel recanalization.

Stroke is a leading cause of disability and death worldwide, with acute ischemic stroke accounting for most cases. Mechanical thrombectomy is a widely accepted treatment modality in appropriately selected patients, demonstrating improved functional outcomes through safe and effective recanalization. However, traditional trials have focused on a narrow subset of patients, limiting its applicability to diverse populations who would otherwise benefit from thrombectomy.

Stroke, which leads to death and disability in high proportions globally, is one of the most deleterious neurological diseases. Ischemic stroke (IS) is the major cause of disease attack and accounts for ~70% of all incident stroke cases in China. Up to now, only two therapies for IS were officially approved, which are intravenous administration of recombinant tissue-plasminogen activator (rt-PA) and endovascular mechanical thrombectomy to rapidly recanalize the occluded artery, which both recanalize the occluded artery rapidly to reduce disability, but are limited in a fixed time window.

Mechanical thrombectomy using stent retrievers is the standard endovascular treatment for acute ischaemic stroke due to large vessel occlusion. With the rapid evolution in device design, this scoping review aims to map the current landscape of stent retriever types used in clinical practice, as well as their application across different sites of vessel occlusion. A comprehensive literature search was conducted using MEDLINE, Embase, and the Cochrane Central Register of Controlled Trials to identify primary studies reporting on the safety and efficacy of stent retrievers in mechanical thrombectomy for acute stroke.
