Article Abstract

In a retrospective analysis of childhood thyroid nodules, 18% were radiographic incidentalomas and 41% were discovered by a clinician's palpation; 40% were discovered by patients' families. The latter group had the largest nodules and highest rates of thyroid cancer metastasis, suggesting opportunities for earlier detection through annual well-child visits.

The ectopic intrathyroidal thymus (EIT) is located anywhere in the thyroid gland along the developmental pathway of thymic descent due to thymic migration during embryogenesis. Ultrasonographic findings of papillary thyroid carcinoma (PTC) resemble those of EIT, which is frequently found in children. We comprehensively evaluated the clinical factors associated with EIT to understand its physiological implications and to explore helpful information for clinical discrimination between EIT and PTC.

Background: Primary hypothyroidism is characterized by a loss of thyroxine feedback inhibition and an increase in thyrotropin-releasing hormone (TRH) levels, resulting in reactive pituitary hyperplasia. However, it is important to note that pituitary hyperplasia due to primary hypothyroidism (PHPH) is rare, particularly when symptoms of pituitary mass compression are present.

Papillary thyroid carcinoma (PTC) incidence increased after childhood exposure to radioactive fallout from the Chornobyl accident. We investigated PTC genomic profiles to distinguish radiation-induced versus sporadic oncogenic drivers by modeling dose and molecular characteristics by driver category: ( = 132), RAS mutation ( = 31), fusions generated from two breakpoints and <20 base pairs (bp) breakpoint gain/loss (Fusion; = 63), or ≥3 breakpoints and ≥1000 bp breakpoint loss ( = 20). The frequency of Fusion-PTC increased with increasing thyroid radiation dose, whereas all others declined.
